Creatintine level post kedney cancer

My grandmother had kidney cancer about five years ago. She is now 87, in the past month her creatintine levelhas gone form 1.8 to 2.0. Her last test showed a level of 2.47. How far from kidney failure is she and what can we do to prevent it form occuring?
