Adult bed wetting in males

  Dear Doctor
  My husband is 40 years old, healthy and active.  Five times in the last three months he has wet the bed.  He is very embarrassed about this and refuses to discuss it with me or see a doctor.  Is there any information you can provide?  Is this usually a medical or psychological problem?  I understand that you can not diagnose this problem over the net, but I would appreciate any insight you may have.  
  Hopefully, if I can just leave your response out for him to read, he might feel encouraged to see a doctor.
  My husband does not discuss feelings or emotions and I believe he is frustrated and angry on the inside.

Dear Ariel,
Wetting the bed can be referred to as nocturnal enuresis.  If your husband has always had this problem, then it would be referred to as primary nocturnal enuresis; this is usually seen in children.  A visit to the urologist is warranted at this age.  A history, physical examination, and urinanalysis should be performed.  In the medical history, questions regarding daytime leakage and voiding pattern will be asked.  The physical exam will include the abdomen and male genitalia including the prostate gland. The causes of the problem are varied including neurologic disorders, infections, bladder lesions and prostate enlargement.
Once the evaluation is completed treatments may include behavioral changes (decreasing fluid intake in the evening), medical treatments, or surgical intervention.
