Already STD tested, something else?

My boyfriend is experiencing every once in a while tingling/ichy/pain in his penis. He says that sometimes it hurts when he urinates. We both had gotten tested for STDs and neither of us have any. At the time that he noticed the tingling and pain I had a UTI and thought I gave it to him. He got antibiotics from the doctor. It went away, but its bothering him again. He also has found a small red dot on the shaft of his penis. It went away before but it came back, and he said it was raised last time. I told him it was a pimple. He had also had an ultra sound done on his testicles, he found a lump in one, but the doctors found nothing just a cyst, no cancer or anything. He is a landscaper and works long days, sweating a lot. I said he might just be dehydrated or that he is holding urination for to long. We do not use condoms, I am on the pill, but we do use KY-Jelly lubricant, that might cause irritation for him? I am also very small and tight, if that could be causing him irritation to?? If there is any information that you could give to me, about what might be wrong and what we might be able to do, it would be greatly appreciated. Thank you.

I would investigate the prostate to ensure infection has been appropriately ruled out.  A rectal exam and/or a transrectal ultrasound can be considered.

Irritation, such as dermatitis, can be considered if infection has been ruled out.  A dermatologist can investigate this possibility.  

Anatomical abnormalities such as a urethral stricture or polyp can lead to burning.  A cystoscopy can be considered if this is suspected.

These options can be discussed with his urologist or personal physician.
