Bacterial prostatitis best approach

Hi All:

I’m writing to you all to share my difficult situation in combating Chronic Prostatitis.

Two years ago during my stay in north of Thailand I had an exposure by receiving unprotected oral sex.

Four days later I felt burning sensation on top of my penis and in urinary tract, one week later a milky discharge also observed and I went to clinic.

I was diagnosed with NSU (Non Specific Urethritis) by picking a sample from inside of my penis and looking at it under the microscope the doctor prescribed 1g of Azithromycin (single dose), one week after taking Azithromycin I was panicked because of no relief of symptoms and took Doxycycline 100mg twice a day for one week by myself, discharge disappeared but the burning sensation was there I went back to my clinic and same doctor tested my urinary tract sample under the microscope and said there is nothing unusual and it is all related to anxiety.

My horrible journey to the world of STD diseases had just begun, below is the list of all the medication that I have gone through till now:

1. Azithromycin : single dose 1g.

That was the only antibiotic that the unknown pathogen in my urinary tract was sensitive to, I remember the second time that I took Azithromycin all my symptoms went away and I felt like new born child but unfortunately I could not abstain from sex, so few days later, after a PROTECTED anal sex I felt that burning sensation back again that was spreading to my testicles.

2. Again Azithromycin : single dose 2g followed 1gr on second day.

I felt symptom free each time of taking Zithromax, but PROTECTED anal sex brought the symptoms back.

3. Ciprofloxacin : 500mg twice daily for 30 days

I broke up of with my girlfriend just due to my suspicion that I might get infected during our PROTECTED anal sex. I was doubting that there must be some kind of bacteria that could be transfered even when condoms are used. I went to urologist with this symptoms : penis skin discomfort and sensitivity, back pain, urgency to urinate but no testicle pain anymore, and after he examined all the evidence he diagnosed me with chronic prostatitis!

It seemed that the unknown bacteria could find his way up to my prostate gland !

4. Doxycycline : 100mg twice a day + Tamsulosin (Alpha blocker) for 3 months :

The one month Cipro regime was not helpful at all, the discomfort and burning sensation on my penis skin , back pain, urgency to urinate and bladder pain forced me to visit another urologist and he prescribed 3 months of doxycycline ! and I took it all. no sex any more just masturbation to help prostate fluid come out,still no avail.

It was on this point that I lost my trust in doctors I was desperate and totally exhausted, ready to get my own hands dirty in the world of health-care and medications,  I started readings tons of articles on the web, I realized that I could not host any Aerobic gram-negative microorganisms in my prostate glad because those Fluoroquinolones and macrolide antibiotics (like Azithromycin) can eradicate them, so I focused on gram-positive microorganisms, I realized that none of the antibiotics that I took was strong enough on gram-positive pathogens (except Ciprofloxacin which many gram-positive strains are only moderately susceptible.) also due to my anal sex habit there was a great chance of having Enterococcus faecalis in my prostate and Augmentin seemed a good choice to eradicate Enterococcus faecalis.

5. Augmentin (Co-Amoxivlac) : 625mg 3 times per day for two weeks.

I had Augmentin for complete two weeks and bingo the symptoms had gone, but 3 days later It slowly came back and ruined all my excitement of the victory. I guess I did not take the Augmentin long enough, so after one week research I put myself on heavier regime.

6. Ceftriaxon IV : 1g per day for 5 days + Augmentin : 625mg + Amoxicillin 250mg twice a day for 6 weeks
E. faecalis is resistant to cephalosporins (e.g. Ceftriaxon) but a cephalosporin will assure me that there won’t be any gram-negative microorganisms in my prostate and Amoxicillin has a good  sensitivity to E. faecalis.

I am still in doubt about the penetration ability of Augmentin into prostate gland but because of my last positive experience with Augmentin I am willing to try it again.

I am on the third day of the regime above and it seems it is working ! (Although lots of doctors told me my reliefs are just due to anti-inflammation of those antibiotics that I take but I simply don’t believe it, I am dead sure that there is a bacteria in my prostate.)

I am taking lots of fluid (5 t0 8 liters per day) to prevent kidney and liver failure and also Phenazopyridine 100mg 3 times per day along with Ibuprofen 400mg to get rid of pain.

Any insight and suggestions are very welcome.
I will let you guys know if I could manage to win this battle.

Some of these comments honestly **** me off a little bit.  Comments that say things like "it ruined my life" and "it will never go away" or "huge depression"... These are not Prostatitis symptoms.  I assure you all I mean no offense, because I know the pain you feel, and I know you're not imagining it, and I know you're frustrated with doctors and the endless courses of antibiotics they keep prescribing you.  All to no avail.  It's immensely frustrating, I know.    But how is your mourning going to help anyone else recover?  
Fighting prostatitis is as much as a mental battle as it is a physical one.  For the record, regardless of what some of these comments say, this disease is curable.  If you have prostatitis now and you're already worrying that you might have prostatitis for the rest of your life, you're already telling yourself that tomorrow you'll wake up in pain.  But the truth is, you might not.
There have been people who have been completely cured, and it can happen to any of you if you develop an aggressive, proactive attitude and solid habits to keep symptoms at bay.  I have been struggling with prostatitis for almost a year now, and I'm 20 years old.  Every time I have sex with my girlfriend, I wince afterwards and have to explain to her why.  I never thought that I'd have any sort of problem with my prostate at all, let alone at such a young age.  At first I thought, wow, that's just my luck.  On top of everything else I have to worry about in life, now I have to deal with a burning sensation every time I pee or ***, and find it uncomfortable to sit down or have trouble sleeping.  Why me, why now?  
So far, the most effective way to deal with this disease, in my experience, is to take it one day at a time.  First off, realize that you're not imagining any of it.  Prostatitis is as really as any other sickness, and so are the symptoms it causes.  Take a moment to realize that yes, it's really unfortunate, and yes, it *****.  But do NOT make this annoyance any more damaging to yourself than it actually is.
Prostatitis will not directly cause laziness.
Prostatitis will not directly cause crippling depression.
Prostatitis will not "ruin" any man or woman's sex life.  
But if you allow it to do so, it might.
How you react to a bout of Prostatitis, both physically and mentally but also just as a whole, can drastically improve your chances of feeling much healthier and happier.  With continued persistence and a positive attitude, you might even find yourself completely cured sooner than you think.  
Here is what I've done, over the past year to keep myself in shape, moving forward towards recovery.  At first, my symptoms were almost unbearable.  But now there are days when I hardly notice them:
1.  Be very in tune to how your body reacts to certain treatments.  For example, if you take a course of antibiotics for a couple weeks and it seems to truly help, keep taking the same one as symptoms continue to diminish.  Same thing with diets and how much you drink, watch how your body reacts to what you do, and don't be lazy with it.  
2.  Drink copious amounts of fluids.  I would say, one or two glasses of cranberry juice a day, and a ton of water on top of that.  When you're thirsty, don't go for soda or juice or whatever.  Go straight for water.  It pays off in the long run.  And forget about coffee, sadly... Or at least until you feel better.
3.  Exercise regularly.  Go for morning jogs, or go to the gym more often, or just do whatever exercise routine you're comfortable with.  Keeping yourself moving is important.  Prostatitis gives you another excuse to exercise more often and keep the rest of your body in shape.  Why not take advantage of that?
4.  Work to develop strong thinking habits that will help you fight the disease.   As soon as you catch yourself thinking, "why did this have to happen to me?"  or "why can't I just be healthy like everyone else?" or  "I'm going to be living the rest of my life in pain and there's nobody who can help me."   Instead of relying on others for help, rely mostly on yourself.  This is crucial.  Because the truth is, after a certain point there's only so much any doctor can do.  Plus, there are far worse diseases out there.  Steve Jobs fought Pancreatic for 8 years, and yet somehow the thought of Prostatitis = doom on some of these forums.  Instead of repeatedly asking the question why and dwelling on your pain and symptoms, tell yourself that you're going to beat it, every day.  Because you can.  Instead of asking yourself, "why me", at every turn, look around a realize all that you still have.  Prostatitis is a challenging life experience, and although it may not be a pleasant one, it can teach you things: how to truly take care of yourself, mind and body; how to not take anything for granted.  
If you do all of these things, and constantly put forth effort in treating and working to overcome your physical symptoms, they are likely to improve drastically.  It's not something that will be easy... Far harder than lying down and giving up.  But it's the best way to go about fighting Prostatitis.  If you're reading this and you've been suffering, I hope that my words can help you believe that there still may very well come a day when you find yourself even healthier than ever before.  Regardless of your situation, there is hope for you.

There has been multiple antibiotic resistant mycoplasma genitalium since the early 2000s. Regardless of what any doctor tells you all prostatitis is caused by bacteria and can be cured with the right antibiotics. I was infected in 2002 still in high school via a woman from Australia who performed oral sex on me. All the symptoms of urethitis initially but was repeatedly told I was negative for STDs nor would they humor me and just give antibiotics. Had they azithromycin would have killed it at the time but by the time I was given long term azithromycin I had already taken and failed doxycycline and azithromycin because I was not on a long enough regiment so it became antibiotic resistant. Cipro does not penetrate the prostate so if it has become prostatitis like mine did after about a year infected it did nothing fast forward 16 years 10 of which I ignored my chronic prostatitis until an ex girlfriend of mine infected a man in th British army. A mutual friend told me after he noticed I was in the bathroom 20 mins he said what are u doing taking a big dump at my house I'm like no I have trouble peeing have prostatitis my friend commented how my ex gave someone something called mycoplasma I knew right then and there that's what had been ailing me all these years. His was cured with pristinamycin mine with moxifloxacin along with ricampcin. Success after sixteen years of agony. I was also very sexually active because jerking off hurt when I came but thru sex or if I took speed or cocaine and built enough pressure I was able to *** in a stream rather than it leak out after I go limp .. also I tested negative for mycoplasma genitalium 2x before testing positive the reason being is multi drug resistant mg doesn't have the same genes that are used as the marker for normal mg. Some labs have the test for Macrolide resistant strains but none that test for the fluroquinonone resistant strains reliably. Good luck and antibiotics will cure it once you find the right one you will know because on day three or four symptoms will be gone. I cried the first time I came as an adult that didn't burn and ache afterwards and started screaming I knew I was right I knew I was right all these ******* years and nobody would listen to me. If a patient says they think they have an STD and if they say any real symptoms such as discharge or discomfort after ejaculation it isn't in their head esp if after a sexual experience. STDs are very very common and sexual active people are more likely to be infected with at least 1 rather than none.

What was your moxifloxacin regimen? 400 MG I'm assuming, but for how long?
I live in the United States so pristinamycin is unobtainable for me, but I am able to get moxifloxacin so I'm quite curious about how long you had to take this medication to experience relief. I've been suffering for three years now

I took Moxifloxacin 500mg 10 weeks for chronic prostatits and it is gone for 3 years.
I also have to mention that working out and staying active and healthy made a great difference.

Ok another update:

I actually forget about this site and the post I made here in 2011, until someone usually sends me a message and asks about my current situation which  triggers as a reminder that I have posted in this website. Anyway I  just wanna update all of you and future readers that I am still having the pain, and all the symptoms, except they are kinda hardened in body and it seems I got used  to the constant pain.

Still suffering for complete 7 years. Someone says I should use Bacterim alongside of other antibiotics for very  very long time, I don't really want to kill my kidney or damage my joints by taking long term cephalosporins.

I'm also fighting this issue. I started noticing symptoms about 2 months ago and have tried Flagyl and Cipro. If I open my urethra I can see very light redness and I'm at a total loss on what could be causing it. I'm going to the urologist tomorrow and am going to see what they can figure out about it.

I've read in other forums that Diflucan has helped with the sensitivity of the glans due to a yeast infection (Candadiasis). Any help would be greatly appreciated.

I too have had an E. Faecalis infection in my prostate and right testicle for 2 years now. Not much help from abx, considering the China clinic but I'll need to save before I can go. Any success stories yet? I'm still battling.

I'm sorry but if you think its possible to go on " jogs" or relax and be hopeful then you dont have a bad case of prostitis. If you had it bad you would be curled up in a fetal position praying for it to end.

My dear friends. I have been suffering with this prostatic nightmare for over 3 yrs now. I have done more research and know much more than any doctor urologist and let me tell you a few facts. 1. It is always caused by a bacteria (why?) because after 3 yrs and then thousand different antibiotics e.g. cipro, norflaxacin, oflaxacin, gentamacin, amoxiclav etc etc. the gooddamn pathogen came out in my semen. 2. It is somehow linked to muscle spasms perhaps spinal cord herniations (I have them). 3. Stress is a major contributor. All in all there really isn't any treatment that would eradicate this bacteria, as we dont really know why our bodies dont fight it off. 4. Case of immunity plays a role as supposedly the prostate has antibacterial mechanisms which should eliminate our bacterias. As far as today medicine and doctors have no answers to our questions and that's the bottom line, so my suggestion is, have an antibiotic course handy and only use it if you are peeing every hour and cant sleep or forgive my french shieeet hits the fan!!! Otherwise learn to live with this disease cause it AINT GOING AWAY EVER !!!

The first thing that you need to do is change your life.  I know this is hard, but everyone here needs to think about what's really important in their lives and realize that having sex with prostitutes and all that is not going to lead to anything good.  So if that's what's important to you then you need to change that and find something else important to you so you don't keep going back down the same road, like an addict.  This lifestyle is hard to change, and it will keep coming back to haunt you but you need to be strong and change your life for good and never go back.  This is very hard, because being sexually satisfied feels very important.  But you have to find other ways of feeling satisfied.  And once you are in a good place with someone who understands your problem, you will have to look for satisfaction there.  This is very hard but you have to realize that sexual satisfaction is not the only thing in life.  You have to realize that this is an addiction, and that going back down that road is like sticking a needle in your arm again.  I live in a place where there are cheap prostitutes everywhere and you just can't give in.  You might think it's okay, because you are using protection, but one day you will forget to use protection or it will fail and you will be back in the same hell.  You need to change your life and just like the 99% of other men, just don't do it.

Okay so now about health and healing.  

I had the same problem, and I got help and I got better.

I think that everyone has to remember that this is a holistic problem, and one drug may not cure it, and it will be difficult.  I recommend going to a naturopath because you may not just be able to zap the bacteria like you can in a simple sinus infection, and then just go back to your old life.  You need to take a holistic approach to this.  I can't stress this enough.  Your whole body needs to be working right in order for this problem to go away or at least become something that doesn't cause problems for you in your life.  

I had a similar problem and I went to Dr. Guercini in Rome.  It was money well spent.  But I can't say that this is for everybody.  I had a relatively mild prostatitis, so it worked for me 100 percent.  Mine was caused by an STD, and I wanted to be cured of that.

I learned a lot from him.  Mainly that the prostate, penis, testes, are a huge bundle of nerves.  And that pain in the tip of the penis often comes from a link to the prostate.  

In my case I got Trichomonas, and that comes with other things.  I got diagnosed by a very wise lab technician who examined the very small drop of fluid that I had every morning at the tip of my ****.  He did a "wet prep" and heated the sample up to body temperature so be able to see the parasites moving.  Without that I would have never known, because everybody says that trichomonas is not a factor for men.  In your case this might be a factor because trichomonas can bring other bacteria.  All kinds of bacteria are normal in the human body, but they have to be balanced out and contained in the right places.  Parasites can change that balance.  That's what happened to me.  I got e coli and eterococcus in the prostate and these things were coming from my own body.

I think there are two options for everybody.  Either go all out on the holistic approach, or go all out on the medical approach, with direct prostate injections.  Or preferably both.  Just do it all, be thankful that you don't have HIV, and change your life completely and permanently.  Just admit that you have a problem and just forget about that mindset that brought you there.

When you get better, keep reminding yourself how fortunate you are and don't go back down the same road.  Because one day you will feel 100%, and you will be tempted to let this happen again.

Have you heard about the Avelox? Those doctors just do not know how to cure the chronic prostatitis. So they say you are fine.

just an update:

I have given up all medications and back to normal life, the symptoms are still there but they are tolerable. some days the pain is in its peak and I have no choice but to take painkillers and rest.

I don't think that I can do anything to go back to normal situation. It is almost 5 years I am suffering. lol

By the way the last antibiotic that I tried and really reduced the symptoms was Levofloxacin which I took along side of prostate massage.

WOW! You guys are fighting some tough battles, and y'all aren't going to give up. I had prostatitis a few months ago, though I'm not certain it's cleared up. In fact, I think I may want to start some treatment for E.Coli. Sex is a beautiful thing, and it can also be an opening for a life of suffering. Human beings are so nasty. Stay faithful guys, and use protection, always!

I seem to have some similar situation going on, but mine is more recent yet still with lingering symptoms . I have taken these meds:

Ciprofloxacin 500mg x 2 for 14 days,  x 1  for 7 days  Seemed to help but I had side effects after 1 week and some symptoms still persisted, such as tingling sensation upon urination.  

Flagyl 2g x 1   Seemed to help but one time dose - effects short lived and also was taken during Cipro regimen so not sure how effective it was.

Doxycycline 100mg x 2 for 7 days   Seems to help, almost halfway through currently, but not sure if it is due to the anti inflammatory effects of the abx.

I did not try public clinics but I did have urinalysis, urine culture, urine NAATs, blood tests - all, waiting on more tests. I have not had semen or blood cultures but I am open to other testing and suggestions, anyone?

I read about ureaplasma but the dr. said they don't know what causes NSU and she did not seem to be eager to order any other test currently.  A urinary tract sample does not sound like a pleasant experience!!  

I had initially urethral itching then irritation followed by burning upon urination and eventually sore/swollen testicles, low grade fever.    By the time I went for treatment it had affected my prostate.  Anyone know how good Doxy penetrates the prostate?

Thanks for the previous comments, gives me some information to think about, but any updates on how anyone is doing currently or any advice would be truly appreciated.

You should all run a PCR test and test for ureaplasma urealyticum - google it, read the articles and talk to your MDs. A stamey-mears test should be made in your case!

I had this same thing going no to after I had unprotected oral sex this has to be a std something that's new because it seems like these Dr. Has no clue whats wrong with us has anyone found a solution to this problem? Cause I'm afraid that I could pass this to my wife I need help. My Dr. Pretty much said there's nothing more he can do he had on cipro, dox, bac and bout three more different ones. So if anyone find a cure please post it fellas thanx

I have had all the above symptoms for the past 5 years,,, all after receiving unprotected oral sex.. I was on EVERY antibiotic known to man , had every test EVERY Dr said that there was nothing wrong except in my mind. I was finally diagnosed with an ecoli infection that went untreated for over a year.. was given Flgyl retested and no infection detected.  EVERYTIME after sex/ejaculation it causes an intense burning sensation and the opning of my urethra gets real inflammed/ irritated... been to several Drs and still saying no infection of any kind... every day is a nightmare for me with the itching, stinging sensation, its as if the very tip of my penis is super sensative and easily irritated.  I have completely lost all confidence in the Drs, they tell me it is chronic prostatits and that my encounter has nothing to do with my diagnosis.. I call ********, and say they just do not know what is going on with me.. it has caused MAJOR depression, to this day my prostate is HUGE, has constant spasms, and is causing me major pain.... I am only 41

hi boyblue i have the same bacteria e coli but for me itwas easily detected in urine culture and whit the prostatic tact it was confirmed that i have bacterial prostatitis mi dr send me cefuroxime 20 days the bacteria is 2.00 sensitive that i think its not much but still have hope that the treatment works hope that you are doing better rigth now, Did you manage to kill the bacteria ?

hey man, i have the same condition since 2003. my symptoms come back almost every time after sex. I have taken the following meds during the last 8 years:

Azithromycin 2g

Flagyl 2g

Doxycycline 100mg x 2 for 10/14/28 days

Rocephin shot

Ofloxacin 200mg x 2 for 10/14 days

Erthythromycin 100mg x 4 for 10/14 days

Ciprofloxacin 500mg x 2 for 14/21 days

The public clinic / hospital docs are useless, they did ultrasound, swab test, urine test, NAATs and kept saying I was fine. Until last month, a private specialist ordered a semen culture and the test came back Strep B. I am currently on Augmentin 675mg x 2 for 14 days. Symptoms came back on the 8th day, I am quite freaked out if Augmentin is not working but on the test report it shows the bacteria is suscepticle to Augmentin/Rocephin/Cefuroxime/Clindamycin. Same here, I am not exactly confident how well Augmentin penetrates the prostate. So far, ofloxacin has been the most effective for me but its expensive so I hadn't took it for more than 14 days.

I need help too man, from anybody who has successfully recovered.

I had the same symptoms and almost the same antibiotics. However, one day I started a 14 treatment for H pylori (causes ulcers in the stomach) with amoxicillin and clarithromycin  mg twice daily with some stomach settling meds and as side benefit, was cured .  For the first time in years, my prostatitis was 100% gone.  Look on line for new research into H pylori infecting the prostate.

hey I have the similar symptoms, I have taken azithromycin 1g dose symptoms disappeared and came back after a few days I took 2g again as a single dose.
I had irritation in urethra and pain in testicles on one side, no discharge or warts or the burning sensation.
And my doc says im fine im just thinking too much about it. I want to know if you were experiencing the same?
I think I might have Prostatitis from the unprotected anal sex with a working girl. :(
Could you also tell me how did your doc. find out that you have prostatitis?
