It will be difficult to diagnose over the internet, considering you have been to 10 doctors already.
A urine test as well as ultrasound of the scrotum can evaluate for the possibility of epididymitis. I'm sure that this was considered already.
Another option would be to consider a transrectal ultrasound of the prostate. This can evaluate for a prostatic abscess - which antibiotics will not treat appropriately.
Interstitial cystitis can be diagnosed with a potassium sensitivity test. If positive, medications like Elmiron can be considered.
These options can be discussed with your personal physician - or in conjunction with another urology opinion.
