You may have a sac of fluid within your scrotal sac, possibly surrounding the testicle. This is called a hydrocele. Usually this is not a serious situation. This can distort the skin and show through tight pants, pull the penis to that side when it is not erect or maybe even bury the penis in the skin. A hernia and a spermatocele are other common possibilities. All of these can be readily treated with surgery. I think that you should see a urologist and obtain an ultrasound of your scrotum to start. These conditions may get worse over time. A principle reason to get this evaluated, however, is to rule out the unlikely situation where something serious is happening (such as a tumor). Again, this last problem is unlikely, given that you have had the enlargement for a long period of time without any other problems (at least you did not mention any).
