Blood in Urine

I am a 41 yo male from the Philippines, a bachelor and working in Saudi Arabia.  On December 23, I was horny and jerked off in the morning; at nightime still feeling horny and jerked off before going to bed.  When i came, i felt a bit pain in my penis.  When I woke up the following day, a blood clot passed out and there was blood in my urine.  Since then, I have noticed presence of blood sometimes in big amount in my urine especially when I wake up in the morning.  There is a pain, a slight pain in my lower back but I dont feel any pain when urinating.  Though the blood is really scary when urinating!  Does this mean I have torn my urethra?  What medicine should I take then?  

Note that this is the 3rd time (I guess) it happened.  The first one was back in 2000, 2nd around 2005!  Doctors here prescribed spasmo-urgenin and I was advised that it will go away in days.  Doctors said that I must have indeed torn my urethra and no big deal to be alarmed.  During the earlier episodes, blood is also present in my semen.  But after a week it did go away!
