Blood in urine, pain during sex, and testicular pain??

My boyfriend has blood in his urine, testicular pain and today while we were having sex he also had pain. This has never happend to him before and is in great pain. He says his testicals hurt and his peanus hurts when he urinates. The pain has been going on for about 2 hours. HE WILL NOT SEE A DOCTOR!
He drinks 10-15 beers a day if not more. He eats very seldomly and smokes ciggarettes. I dont know if any of this plays a role in what is going on but i thought it might be valueable information.
