There can be many causes of blood in the urine, including various prostate problems (infection or cancer), bladder diseases (including cancer), or kidney disease. A varicose vein rupturing is also possible.
There has already been a pretty comprehensive evaluation including cystoscopy and IVP.
I would consider repeating the cystoscopy to ensure there isn't any evidence of bladder cancer. A prostate exam and PSA can be considered to evaluate for prostate cancer. You can also consider a kidney ultrasound to evaluate for kidney disease.
