CONFUSED WITH PENILE HEAD/MEATUS IRRITATION

This all started after what i can narrow down to either unprotected oral with my GF OR using antibacterial soap on my genitals after working out at the gym,SPECIFIC symptoms burning feeling on penis head when rubbing on underwear,irritated meatus looks puffy sometimes especially after mastubation(circumcised) is been going on for almost 10 months now,2 std panels all negative,My PCP didnt think it was HSV so never tested for it,Im positive for HSV1,prior cold soars as a kid,he gave me triamcinolone cream .1% PRN ,seemed to help im afraid to use for long time,saw urologist examined my penis said looks normal,U/A negative,DRE prostate feels normal,saw dermatologist took a quick look at my penis,he thinks is penile eczema gave me desonide 0.05% bid x 2wks,i got say i have seen alot of improvement with this TX,for the first time in 10 months i can wear boxer shorts,but i still get occasional irritation which are driving me crazy,MY questions 1; could this be NGU with all negative U/A,2;could this be fungal without obvious whitish looking fungal apperance,3;could this be me noticing any sensitivity that could be overlooked if i didnt pay to much attention,4; do i need a penile ultrasound to check for maybe traumatrized veins or tissue on penis.THANKS U INPUT WILL BE APPRECIATED.

Thanks for posting your query.

As we had discussed earlier on the dermatology expert forum also, it is less likely to be caused by any STD and is most likely caused by chafing skin or due to dermatitis skin. To answer your questions one by one:

1) It is less likely non gonococcal urethritis which mostly causes dysuria, a white/cloudy discharge and a feeling that one needs to pass urine frequently, discharge from the penis, burning or pain when urinating, itching, irritation, or tenderness, and underwear stain. It can be diagnosed easily by swab test or by urine test. But false negative test results can also be there. Hence it would be wise to repeat the tests. Else you can opt for specific tests for Chlamydia which include Nucleic acid amplification tests (NAAT, to find the genetic material of Chlamydia), Nucleic acid hybridization tests (DNA probe test, Enzyme-linked immunosorbent assay (ELISA, EIA), Direct fluorescent antibody test (DFA) or chlamydia culture.

2) It does not look like a fungal infection and moreover it is responding well to steroid ointment which is less likely in case of a fungal infection which gets exacerbated with use of steroids.

3) Your apprehension and checking the penis regularly and frequently can be responsible for the sensitivity in the penis area. Stress and anxiety often cause feeling unusual in skin like crawling, numbness, tingling or loss of sensitivity.

4) With only burning sensation on penis and puffy looking tip of penis as main symptoms, there is not much of a need for a penis ultrasound based on these symptoms alone. But to completely rule out any prostatitis, get a testicular and penis ultrasound and prostatic fluid examination along with prostate specific antigen.

Follow the advice given in the earlier post and apply the steroid cream only under prescription. It is most likely to be chafing or dermatitis skin and should get better with washing, application of calamine and topical steroids and with less manipulation of the skin of penis due to anxiety.

I here u men ,this situation is real problem ,but no medical tests out there seems to identify the cause,i have done my research i have come down to;
1;during oral sex some how adenovirus got into the urethra did some damage that is taking long to heal due to acidty of the urine.
2;pelvic muscle spasms that flare up causing meatal inflamation
3;bacterial/fungal during oral that colonised the opening of the urethra
4;skin chafing that wont heal properly second to constant rubbing.
5;Anxiety has got the best of us,is in bedded now in our brains
6;HPV other than the one causing warts,HPV 16 in males need to be researched
7;Herpes either 1or 2 shedding without obvious blisters

Im in the same boat, negative std test(multiple times) dermatologist said things look normal, urologist said things look normal but the meatus didnt feel normal whstever that means. Dx as proststitis, which i dont fully agree with since my only symptoms are red irritated meatus with the apperance of puffy skin. It makes me so mad to hear 2 drs say it looks normal after seeing my penis for 2 seconds. I kno this isnt normal for me since ive been looking and playing with the god damn thing for 26 years! We need answers man!!! Hopefully things start looking brighter for us in this terrible situation.
