Chronic testicular/genitle pain

In 2002 I had a surgery for a varicocele on the left side of my scrotum,
as a result a hydrocele developed. In June 2009 I started getting a lot
of pain on the same side as the varicocele, so I went to see a doctor
for a diagnosis. It was obvious that the hyrdocele had gotten noticeable
larger in size. The doctor suggested that it was the cause of the pain
and performed a surgery to remove it. Now it is October 2009 and I am
in twice as much pain than before either surgery. The pain constant
24/7 usually on the left side of my scrotum/left testicle it is hard to
tell. Sometimes the pain radiates throughout my entire genitle area, at times
making it hard to pinpoint the origin. The pain is a throbbing pain
that is accompanied with a burn and a sensation that you would get when
your foot falls asleep. I have noticed a small lump over my left
tesicle but it is not attached to it. I have visited three doctors to
find out what is happening. They usually run the same test (palpation,
ultra sounds and urine test for infections) but all say that everything
is normal. THIS IS NOT NORMAL. They usually prescribe the same medicine
for a possible epididymitis and Motrin or Tylenol for the pain. NOTHING
HELPS. This pain is ongoing 24/7 making it very difficult to live a
normal life. I am really desperate. Do you have any ideas or possible
suggestions of what could be going on with me? I would really
appreciate your time and help. Please, can you help?

I know that apart from the physical examination of the scrotum and ultrasound , there’s no other modality to know the cause of the pain. However, it could possibly be chronic epididymitis which can cause the pain.
