Vesicoureteral reflux obstruction from stone fragments, infection, and temporary decreased kidney function are all possible side effects. A hematoma (blood clot) around the kidney after ESWL is not uncommon, but these are usually of no consequence.
In your case, you have persistent blood in the urine and several studies have been performed. Sometimes an injury to the blood vessels in the kidney occurs, and this can be diagnosed on a renal arteriogram. This x-ray test of the kidney blood vessels is somewhat invasive, but if a leaking blood vessel is identified it can be closed off during the same procedure (embolization). You should also be evaluated for any clotting abnormalities (platelets, clotting factors, blood thinning medications etc.).
