Testicular cancer usually presents as a firm mass. Most are non-moveable, but atypically, some can be moved. The scrotal ultrasound should be able to differentiate whether this is cancer or not.
It is possible for a scrotal pearl to enlarge over time. Other possibilities for a scrotal mass can include a cyst, hydrocele or varicocele. These possibilities should be able to be seen on ultrasound.
If the symptoms worsen, then this can be surgically removed. I am not a urologist - so I cannot give exact details for the procedure this would entail.
