ED and Discomfort

I've been trying to figure out whats up with me for a while, about a year or so. 40 years old, I've had occassional ED which seems to be progressing. Also, left testicle feeling like its not quite sitting right.  It seems to retract upward and further to the left often and is tender.  Right one doesn't.

A few days ago, went to Urologist and he tried to get prostatic fluid but couldn't obtain any.  Since then there has been fresh blood in my ejaculate.  I called him and he said it may have been from a burst blood vessel while he was trying to get fluid to test.

Now my scrotum feels a bit inflamed and my penis feels a little cold, like poor circulation.

Would it be a sign more of infection or more of circulation if during arousal one's penis engorged a bit at a time from the base upward?

Neither urologist nor I felt a hernia, but he suggested I see a surgeon to look for this.  This is what I'm planning to do but I'm also worried about the current symptoms.

Imaging the testicle at this time would be appropriate.  This can look for any anatomical abnormalities (i.e. torsion) as well as infection, cysts, hydroceles or varicoceles.

Regarding the bleeding in the ejaculate - again infection can be considered.  The prostate can also lead to bleeding.  Because of this, a transrectal ultrasound to image the prostate should be considered.

These options can be discussed with your urologist.
