I'm a 22 year old male, with no serious medical history. Aproximatey 2-3 weeks ago i had an strong orgasm. Imideately (after)/during ejaculation there was a sensation of contraction in the area of, what i guess to be the spermatic chord, epididymus or in that area. After this, which was not what i would describe as painfull, but perhaps discomforting, my right testicle was elevated(at a higher position) and enlargened/hardened still without pain. As this didn't go away, i was afraid that it would be testicular torsion and contacted a doctor, who said that unless heavy pain was encountered, i should be calm. Now 3 weeks(give or take a little more/less) i still have these symptons, along with a sensation of pulling/dull extending from right testicle upwards to the right(groin), and perhaps down into the inner leg(although this is hardly notiable and i am therefore not sure at all). The testicle is still at higher position and seems(again not 100% sure) to have turned somewhat. It is also hardened and larger, but still not painfull but discomforting. It moves down at som rate, especially when lying down, and when i do som sort of relaxtion of muscles perhaps, that i am not sure of what exactly is. I point out that this happened acute, and that i have never encountered anything like it. The sensation at first was perhaps like "pulling" some sort of string. I dont believe that there are any kinds of lumps or anomalies except for what i have described.

All the possibilities you described are possible.  I would consider a scrotal ultrasound for further evaluation, and to rule out the possibility of torsion.  

If negative, urine tests for infection, as well as a rectal exam to evaluate for prostatitis should certainly be considered.

I would also consider a referral to a urologist.
