Groin pain - low back pain

Hi,

I have had this problem for just over 2 years now and am
still trying to get to the bottom of it.

During ejaculation with my partner 2 years ago i noticed
that my left testicle had asdenced into my abdomen just
above my left groin. I pushed it back down as it did not
go back into the scrotum on its own and apart from aching
a bit it was OK. That aching went on but slowly faded
after a couple of weeks to be replaced by a burning type
sensation in my perineum made much worse by sitting. I went to
see my doc and he didn't really know but talked of
prostatis and did bloods, urine and a dre etc for this. There was nothing
present. Not knowing why i had this pain in the perineum
he said it would most likely just fade.

I moved house and so went to see a different doctor - They again did nothing to investigate the testicle issue but concentrated on my back. Had MRI's done and they were all clear. Went to physio and again this made no difference. Ended up with a pain guy who did facet joint injections thinking the groin pain was referred from my back. Again this has made no difference so what could my problem be? Basically my symptons are

Pain in perineum when sitting (burning type sensation)
Pain when pressing on the left of my groin where my
testicle ended up (burning type pain)
'Strange' sensation in the area of the 'tubes' between abdomen and scrotum.
Sometimes feel sick but never am
When pain is bacd i often have increased flatulence
The left side of my groin feels 'thicker' than my right

Now doing research on this various things turned up
however what is the thought that this could possibly be a
inguinal hernia of some sort or damage to the various
structures around my groin? I have seen references that
talk of these type of hernias referring pain into the
perineum.

i'm trying to move back to my consultant from the pain guy
to have this checked out more but what do you think as its been going on for a long time without any type of resolution.

Hernia and infection would be the two things that come to my mind.  To evaluate for a hernia, I would suggest a referral to a urologist or general surgeon to examine whether a hernia is actually present.  If so, it can be surgically repaired.

Infection of the prostate can cause the perineal discomfort.  A urologist should be able to thoroughly examine the prostate and any prostatic secretions can be sent for analysis and culture.  If the results are non-diagnostic, a transrectal ultrasound can be considered for further imaging of the prostate.  

Followup with your personal physician is essential.
