Grossly enlarged right testi and grape sized left testi

My right testicle is very grossly enlarged as a tennis ball.  My left testicle has shrunk in size to that of a grape.

My left testi doesn't hurt unless I touch it.  It has a dull pain in it.  

My right testi is in severe pain and is shooting sharp pains from the testi to the upper right side of my hip.  My right leg is having muscle spasms more than usual.

I have trouble urinating.  I always have to go and I always have to wait at the toilet for the event to happen.  The urine is hot and burns a bit.  I am frequently going to the restroom at all hours of the day and night.

I am 45 years.  I am a disabled veteran.  I had a parachuting accident  in 1982 and had extensive problems from breaking my back from the fall.  I had a spinal fusion of the lumbar 3,4,5, and S1.  I have muscle spasms in the legs and back from this condition.  I am on oxycodone and methadone and cyclopenzaprine for pain management.

I had this happen to me earlier this year.  I was told I had prostatitis.  I was given antibiotics and the pain and swelling went away.  The testis got a little better in their sizes.

Now I am having the same problems again but I have much more intense pain form it and the testis are grossly misfigured.  I am having severe lower back pain and my anus area hurts as if it is swollen and is giving me great pain.  My right testi is seriously in pain.  It is hard and very abnormal.  

I am currently on Cypro for antibacterial infection.  I am near the end of the medication and the symptoms are very apparent still.  No change in the sizes except that the left testi seems to be shriveling up to almost nothing at all.  I think it is dead and is eventually going to wither away.

What should I do?  My first time, they did an ultrasound and found nothing.  The second time with this and they didn't do any thing to look at it.  They just prescribed the cypro.

What do I need to ask?  What do I need to pay attention to in this situation?

My first question is what did the urine culture show and is the bacteria sensitive to the antibiotic that you are taking? Why do you get urine infections? Are the nerves that allow your bladder to function working properly or were they damaged in the accident? Have you had a vasectomy? (If so, then the pain and swelling may not be related to an infection).
How about a repeat ultrasound if the physician taking care of you cannot be sure that this is an infection issue?
There are several possible causes for the atrophy of your smaller testis. The most obvious to me is that it was previously infected and damaged to the point that it is no longer functioning adequately. Was it damaged in the parachute accident? That may be another reason why it shriveled up.
Talk to your physicians, get some answers.

It sounds like you might have a case of orchitis. It is best to seek medical attention right away.
I had orchitis numerous occasions and eventually needed to have a left emergency orciectomy after a a staph infection in a fistala on my left testicle.

Don't wait at home this can become serious in a short amount of time.

Dear Dr. Liroff,

-After feeling on my testicals/scrotum I've notice that both testicals are being housed on the left side of the scrotum. The other side has the access skin located on the bottom of the penis. When I take the testical closes to the right sack and move it with my fingers I can move it back to right side, but it doesn't stay and ends up on the left side with the left testical. Is this a concern because my wife and I have been trying to conceive and I've been recently diagonosed with Aloosperma or no sperm? This has been this way for a while and I thought maybe it would correct itself. Can you give advise or provide feedback. I did have a problem a while back with varicele veins and the testes ascending in the adobomen. Please advise!!

I went to the VA Emergency Room yesterday.  My pain is greatly intensifying.They did another ultrasound and discovered that the right testicle is packed full of fluid.  The left testicle is enflamed and shrunken.  It is enflamed due to the right one taking over the space.  This is what they told me.They didn't tell me anything else.  They gave me a shot of steroid and a prescription of vicodin and said that a urologists office will contact me in a day or two.My parachuting accident did harm my testicles slightly.  They were in good shape but I did have pain in them.  My L-5 nerve root was smashed and is still smashed.  I have what they told me as having impartial paralesys of the right side.  I do not feel sensation on my right side.  I feel pain but not touch.  The parachuting accident did a lot of inside damage but not life threatening as far as the organs go.As time goes on I am getting more and more issues arrise from the accident.  I am still unable to urinate fully with a weak stream. 

I've had no vasectomy.  My left testicle has always sucked up inside while having sex since the parachuting accident.  My ejaculations are sometimes painfull and burn.  Sometimes they are enjoyable.  And sometimes not.

I do not look forward to having sex because of this.

I have been on Oxycodone for more than 10 years.  I wonder if the pain meds I take contribute to it.

What should I ask the doctor knowing these things now?  You know just as much as I do now.  I am still in great pain and the steroid shot and meds don't help one bit.  I am still suffering with a lower right belly pain when I push on it slightly.  They didn't even worry about that at the emergency room.  I told them about that and I told them that my tail is bruised and hurts when I sit.  It all seems connected in some way.

I need to know what questions I should ask and what thhey need to be told.

I am 82aratrooper.

I went to the VA Emergency Room yesterday and they did another ultrasound.  They determined that I had Hydroseal.  Fluid build up in the right testicle.  They gave me a steroid shot and some pain meds and are making me an appointment to see a urologist.
