Without examining the lesion, it will be impossible to say what you have.
One possibility may be herpes. This can result in recurring genital ulcers that come and go. Other possibilities would include syphillis or chancroid.
Regarding the

Scrotal swelling
Testicle ultrasound swelling, there are several possibilities - including infection, cysts, varicoceles or a mass. I cannot say for sure without examining you.
Options for further evaluation would include a scrotal ultrasound to evaluate for any anatomical diseases (i.e. varicocele, masses, or epididymitis), or blood tests/herpes culture for the diagnosis of genital herpes or syphillis.
