Incontinence in 6 yr. old with Complex Partial Seizures

Dear Doctor:

My son is on Depakote 125mg qid.  He has also been diagnosed with Tourettes.  He initially was started on Tegretol, but a week and a half after starting it he slowly began losing bladder control.  After weaning him off Tegretol after having been on it for two months, he still displayed some incontinence.  The day after starting Depakote he lost all bladder control.  Every day since he's has accidents - only once at night.  He's never aware of it when it happens and sometimes I have to be the one to prompt him to change clothes.  He frequently goes to the bathroom, but stated he continues to dribble afterwards.  His neuro thought this was unrelated to his seizures and thinks it is a urology concern.  His PCP thinks it's a neuro issue, but has referred us to a urologist just to make sure.  It almost seems to me like whatever muscle is holding the urine in is letting go.  I wonder if these seizure medications could have an effect on that muscle.  What are your thoughts?

Dear Dee,
Night time incontinence( or enuresis) due to Depakote is a known side effect of this medicine, but Tegretol does not cause urinary incontinence.  Tegretol tends to cause urinary rention and urgency.  It seems that your son is showing a worsening of his night time incontinence as a result of  Depakote.
The best place to start is to see if your neurologist could prescribe a different anticonvulsant, and if so, whether the night time incontinence resolves or not. If the problem persists, the a visit to a urologist may be in order.  Enuresis usually resolves on its own, but if it persists it may be worked up by a urologist.There tends to be a family predisposition to this condition. Other patients may have a bladder with neurogenic problems or one with a small volume. In any case the treatment approach would consider the potential underlying cause.
