Incontinence???

  I am a 38 year old Mother of two.  My children are teenagers however over the past 10 years, I have had more and more occassions of incontinence.  I was told that Chil bearing can cause this.  I have had a Urodynamic study done and found  that the Urrethra muscle is very weak.  I will be having  sling surgery to correct this.  Now the point of my question.  How is this classified and what do the classifications mean?  I was told I was a something 3 and then something else with  a 3 and sometimes a two.  What does all this mean?  I meant to ask when I had my appointment but,  really was surprised by the findings and didn't think to ask at the time.

It is true that child bearing can have an effect on stress urinary incontinence which from your description is what you have.  Stress urinary incontinence is divided into three categories (primarily divided by the fluoroscopic or radiologic evaluation). The definition of stress urinary incontinence(SUI) denotes involuntary loss of urine when the total intravesical pressure exceeds the maximum urethral pressure in the absence of bladder activity. Type 1, is incontinence without primary sphincter incompetents and on fluorourodynamics(FUDS) a bladder above the pubic symphysis.  Type two SUI is a competent sphincter with a bladder below the pubic symphasis on FUDS.  Type three SUI what you have is an incompetent urinary sphincter(weak muscle).  On the pressure portion of your study, your bladder leaked at a pressure below 60 cmH2O.(some Urologists use a threshold of 90 cmH2O.)
I think a sling is a reasonable approach.  This often will take care of your problem for several years.  Patients often experience the opposite type of problem, going into retention, until their bladders have a chance to compensate.
