Itchy Unary Tract

I'm a 46 year old male and I recently had unprotected sex where the woman I had intercourse with could discharge a lot of fluid during sex. Shortly after a day or so I noticed a slight itch just inside the tip of the penis. I was worried about an STD and went to the urologist. The urine test came back negative but the doctor had started me on doxcycline for 10 days. Spelling is wrong.  The condition did not go away, He gave me two more medications and the condition has not gone away. I went back for a second urine test and that also came back negative for any STD's
Recently I thought it was starting to fade away and resurfaced stronger.  The itch at the beginning was sporadic and was not that strong now it is more constant and stronger. it sometimes occurs after ejaculation.
Any suggestions would be helpful.

I have the same itch in the penis, and a moderately inflamed prostate. All my urine tests for chlamydia have come back negative as well, but my doctor gave me 2 weeks of doxycycline, to be followed by 2 weeks of Lavaquin, for the prostatitis.

After reading this, I'm going to ask for a swab test, and another 4 weeks of Lavaquin.

I've been on the doxy for 7 days and the itch is still there, how long did it take for your itch to go away?

Its 2011 and I think that the Lavaquin worked but recently i had unprotected sex with a new partner. I developed symptoms the next day itchy urethra, went to docs, urine test, Negative, (same dam dream here) went to urologist for persistent symptoms. I wanted a swab test! Most places do urine test now and even a place i went to locally switched to urine. I can't understand this with my situation alone and what seems like problems with everyone. The medical community is sending infected people back out to infect others and telling them there test are negative!
I asked for a swab test because i had insurance they did it and guess what? thats right positive for Chlamydia.
I have to see what the urologist wants to now. I am not very hopeful they have any clue what to do.
I recently found a post on this site for a Dr Attila Toth in NY who has a whole new way of dealing with Chlamydia in the prostate but he is far away, he does not accept insurance so it leaves me with coming out of pocket with what could be over $5000 and traveling to NY from Boston or dealing with doctors here that appear to not have a clue.

Just wanted to update you. I appreciate the help you have given me. I went back to my urologist and asked them to do that test and they do not do it there. The STD clinic I went to has been very helpful but they do not do it either. The nurse practitioner that i have been dealing with there gave me the name of another urologist I can go too but in the meantime I had found my own.
Before I left my last urologist he gave me a months supple of doxycycline. I am down to the last week and still have some small symptoms showing. This week i went to the new urologist he prescribed me lavaquin for 6 weeks. Today he called and said he thinks i should stop and be retested after a week of taking it.
I am skeptical of this and told him well if i still feel symptoms then I know its there.
He can not perform the test you mention either.
I have another appointment with the Boston urologist and may have a hard time getting there there but may try just to see if they can do that test.
I hope that the lavaquin works and i can be done with this.

Hi,

Ciprofloxacin has been contraindicated for the treatment of chlamydia as it is ineffective or only weakly active against this organism.

Please follow what I am posting here carefully:

Go back to your doctor at the clinic and request for a culture sensitivity of the chlamydia organism causing this infection. This is to identify the antibiotic that the organism is sensitive to and so that you do not end up losing more time and added cost on an antibiotic that the organism is resistant to.
This has to be done on a specific culture medium that is used for chlamydia and not the regular medium used for other bacteria.
You can stay on the cipro as it may have weak activity but you will not be cured of this infection by the ciprofloxacin alone.
Prostate infections need to be treated for a minimum of 15-20 days. This is because the prostate is an organ that the antibiotics cannot permeate easily.

Follow the general advice in my previous posts to avoid irritation to the prostate.

Do revert back with any further doubts you have.

Was informed by the clinic that the swab test came back positive for Chlamydia. I am currently on Cippro for 25 days.. All other meds that i have taken so far have not done anything and it may be because the infection is lodged in the prostate.  Have not heard back from urologist to see if he wants to change meds or give me a couple at once since this infection has proved very resistive to standards medications.
Any thoughts or answers would be helpful.

Went back to MGH clinic and discussed my options with them today, they said they have done and I have been treated for everything that they can do there and said that it could be a infection in the prostate and i should go back to my urologist. It seems like I am bouncing around and not getting treatment and they are throwing there hands in the air.
I made another appointment with my urologist tomorrow to see if he can find an infection in my prostate. This has been going on now for 2 months now and it's getting very frustrating.
If it is an infection in the prostate how can he find it and what should he be looking for?  I know when I went there the first time that he did do the standard check of the prostate and said it seemed fine.

I recently went to the Mass General STD clinic to get another opinion for what is going on with me. They did a swab test and found a high white blood count, I told them what I have been treated for already and what the history was and they decided to try the medication for the Trichomonas, Flagyl. I took a large doasge all at once.  
They also called and gave me a large does of Azithromycin 500 mg and i took that to treat some of the other potential problems.
It has been 2 days and it does not have seemed to work. I will go back there for a follow up and let them know what you said in your last email. Is there anything else I should be taking or trying?
I think the urologist checked my prostate but he did not use a scope at all if thats what needs to be done and i have never taken any mends for 14 days.
I will look at the site you gave me and get back to you.

Hi,
Burning sensation intensifying after ejaculation indicates that the infection is based in your prostate gland. Infections in the prostate gland are not usually detected in urinalysis unless the urine sample is collected right after ejaculation or a swab for secretions from the urethra is taken in the morning prior to urination.

More specifically, other infections of the prostate are also usually undetected in urinalysis. This is countered by examining the prostate fluid or semen for infection.

A blood test will reveal changes in the WBC levels and indicate presence of infection with an increase in WBCs.

Organisms like Chlamydia and Mycoplasma are transmitted sexually and are not detected in urinalysis as they are intracellular parasites. There are specific tests that screen for these STDs and you can check if you have been tested for them.

Also a DRE of the prostate gland by your doctor will reveal whether there is any inflammation or enlargement of the prostate.

Also if you have discontinued antibiotic therapy, you may want to discuss restarting antibiotic treatment as prostate infections may require a minimum duration of treatment of about 14 days to be effective.

I went to my urologist and told him to run a test for trichomonas, he informed me that the other urine test i had done usually scan for the this.  We ran another urine test anyway and the results came back negative again. This is the 3rd test and all negative. I still have a tingling/itchy urethra and can't find and answer why. This all stated the day after having unprotected sex.
Does anyone else have any idea on what this could be?  The symptoms are not always present but come and go through the day, this is the only symptom. it seems to be aggravated after ejaculation and symptoms are present more.

Hi,

Itching or tingling sensation during ejaculation or urinating can be due to an infection by a protozoa called trichomonas. Though usually asymptomatic in men, this infection is contracted sexually and can result in constant tickling sensation in the urethra.

The infection can usually be detected by a urine microscopic examination called wet mount.
Treatment involves a metronidazole (Flagyl) 500 mg twice daily for 7 days or a single 2g dose orally.

The infection can be spread to your partners through sexual intercourse in the treatment period, hence, practice safe sex (condoms) to avoid infecting others and contracting other infections.

Drink plenty of fluids (over 2000 ml per day) and avoid caffeine and alcohol while on the metronidazole.
