Kidney Pain

im a male 43 years old. 6 feet, 180 lbs and fit. for the past 11 months im experiencing kidney pain at the right rear flank below the ribcage. sometimes i need a tylenol. other times i dont feel it. also occassionally at the left flank as well but not all the way over to the hip bone. it is much more mild on the left. urinalysis a few months ago showed no blood. renal ultrascan of both sides showed no stones. blood workup came back stellar including PSA for prostate. ive had a couple different periods of urinary sensations i.e. light burning but nothing very uncomfortable and they went away in a few days to a week. sometimes i have periodic sensation in the urinary tract below the scotum but not when urinating. this sensation does not feel to be in or emanate from the scrotum or testicals. urine flow seems normal and regular in color and ive had no difficulties in urinating ever. ive also been drinking a lot of water. the right side will ache when i lay on it sometimes. other times it will not. the left rarely
aches if i lay on it. i usually sleep on my left side or back for that reason. i do not need to get out of bed to urinate at nite nor have i ever really had to. bowel movements are regular and i
feel 100% except for the flank  sensations. i do have periodic episodes with hemmoroids (hemorrhoids) but over the counter creams work quickly. strenuous actvity or lack of it seems to have no effect. thanks ***@****

There may be two different conditions present.  

First, regarding the flank pain, you amy want further evaluation for kidney stones.  Typically, a pelvic CT scan would be a bit better than an ultrasound to detect stones.  I would also check a urinalysis to make sure there is no blood in the urine.  

Regarding the sensation in the urinary tract, I would evaluate for prostatitis.  A urinalysis is recommended again.  A digital rectal exam by a physician to test and culture the prostatic fluid is also recommended.  I would also test for STDs such as gonorrhea and chlamydia.  

Followup with your personal physician is essential.
