Low level of Testosterone & high Prolactin

Male - 29 Years          Single, Indian origin, Vegetarian, No drinking/smoking. Staying alone from plast 7 years. Not taking proper & timely food. In depression due to many personal problems.

Problem 1. Have no hair on beard/chest. Lump on chest found 2 yeas back. On left side it is bigger than of right side.

A MD, Surgon dialogize as Gynecomasita. But when shown to endocrinonologiest they said as only fat. As pe Endo. Mammary gland / gynecomastia lump should be of round shape ONLY. Is it true?

Is any mammogram / Xay available to find out it is gynecomastia or not.


Problem 2. Low level of Testosterone & high Prolactin.

It erect normally but NO eruption of seaman take place. Not kept any record but feel was normal before 2 years.

On chin area have 40-50 hairs only as beard. Once shave take 3-4 days before next shaving is require.

What should be next steps? If MRI of brain of pituitary done, from MRI only can one identify type of tumor eg. Acromegaly / Hypopituitarism etc.

Can this be fully cure without causing other side effects? How much  time it take?

Blood test taken at 10:00 AM. [empty stomach]

2 weeks later 2nd blood test taken. [empty stomach]

Prolactin 191.26 ng/mL
Thyroxine (T4) 7.70 ug/dL
T3 117 ng%
TSH (IRMA) 6.23
Basal Cortisol 15.7 ug/dL

Hi,
It is possible that you do have a pituitary adenoma which is prolactin secreting. The MRI will only visualize it, it is not yet therapy. Therapy is usually though surgery by removing the mass. Other options are radiation and medical therapy. If there are symptoms of visual field defects (put another way, blurred vision on the periphery) this indicates compression on nerves serving the eyes,  surgery may be the best treatment as it will remove the pressure and may save eye functionality.
The prolactin causes a suppressive effect on the other hormones and hence there is decreased testosterone. Once the prolactin suppression is removed, there should be reversal of its effects, however, if the suppression has been long-standing – the gonads may not recover function fully, hence, it is uncertain how complete the reversal would be for a 29 year old.
Discuss your chances with your doctor, a thorough examination would help gauge your chances for complete reversal.
