The cost may vary based on where you are located.
It would be best to ask the hospital you are planning on having the treatment done at as to what their cost would be, inclusive of medications.
You may also want to consider catheter embolization (interventional radiology) as an alternative to varicocelectomy as the costs are significantly lesser.

Perhaps the accumulation of blood causes the testes to be hotter and so damage sperm production; or the pooled blood brims over with abnormal hormones which may change the way the testes make sperm. The effect of the varicocele on an individual's sperm count is variable - and this may range from no effect whatsoever, to causing a decreased sperm count. Varicoceles may also have a progressively damaging effect on sperm production, so that the sperm count may decline with time"

Tests specific to the varicocele include ultrasonography and veinogram.
But as there may be other causes for the low sperm count, you will need to have basic tests, including blood tests, tests for antisperm antibodies, obstruction of blood flow in the testes, hormone tests, etc.
Smoking has been identified as major factor adversely affecting male fertlity.
A grade 3 varicocele, if identified as causing infertility, will need surgical intervention to correct the problem.
However, this will need to be done after evaluating for other causes that may be causing the condition.

my dh was diagnosed with varicicele vane i his left side of scrotum(it is very large but doctor didn't tell him what size it is from "1-4",i learn that online ) so his urologist told him to do the semen analysis which says that he has a low sperm count (13.3)milions,low morphology (which i cant read)and oligoteratospermia...doctor recommended surgery of the vane but we are not sure if that would do any good,can you tel me what tests should we do exept semen anlysis to be sure if varicicele is cousing infertility...

Oligoteratospermia means a reduction in the concentration of sperms and a decrease in the number of normal-shaped sperm.
