PAIN AND DEAD TESTICLE

MY BOYFRIEND IS 28 YRS OLD AND NATIVE AMERICAN.  A COUPLE OF MONTHS AGO HE WAS HAVING A PAIN IN HIS LOWER ABDOMEN THAT WENT DOWN TO HIS RIGHT TESTICLE. WHEN HE SAW THE DOCTOR HE WAS TOLD THAT THE TESTICLE WAS COMPLETELY DEAD.  HE HAD TO REMOVE IT.  THEY DID ALL TEST FOR HIM TO DETERMINE THE CAUSE BUT THEY COULD NOT FIND ANYTHING.  THEY SAID IT COULD BE VASCULITIS BECAUSE HE HAS SOME OF THE SYMPTOMS.  THEY WERE AFRAID TO PUT ON HIM MEDICATION FOR VASCULITIS BECAUSE HE DID NOT HAVE ENOUGH OF THE SYMPTOMS.  A COUPLE OF DAYS AGO HE STARTED HAVING PAIN ON HIS OTHER TESTICLE.  AFTER AN ULTRASOUND THEY SAW THAT SOME OF THE TISSUES ARE DEAD. THEY AUTOMATICALLY PUT HIM ON STEROIDS AND RECOMMENDED THAT HE FREEZES HIS SPERM.  IT IS VERY FRUSTRATING THAT THE DOCTORS ARE NOT ABLE TO KNOW THE CAUSE OF THIS SO THAT HE CAN HAVE THE PROPER TREATMENT.  HE IS IN THE NAVY AND HE HAS BEEN SAYING THE NAVY DOCTORS.  NO ONE IS ABLE TO GIVE US AN ANSWER, ALL THEY CAN SAY IS THAT ITS NOT CANCEROUS, AND THIS IS A WEIRD CASE THEY NEVER SAW BEFORE.  DO YOU HAVE ANY SUGGESTIONS AT THIS POINT?  WE ARE REALLY LOOKING FOR WHAT TO DO NEXT.  WE WERE THINKING THAT IT COULD BE HELPFUL TO SEE A DOCTOR WHO KNOWS ABOUT NATIVE AMERICANS MEDICAL PROBLEMS.  PLEASE GIVE US ANY LEAD AS TO THE BEST WAY TO GO ABOUT THIS.

I would suggest another urology opinion, preferably at a major academic medical center.  

I agree with imaging the scrotum with an ultrasound can be considered to rule out torsion or other anatomical abnormalities.  More imaging with a CT or MRI can be considered if the ultrasound is non-revealing.  

It is difficult to speculate what is going on with examination.  However, if the treatment includes removing the other testicle, I would definitely obtain a second opinion.

Followup with your personal physician is essential.
