Pain at tip of penis

I have had pain in my penis for two weeks. The penis hurts in the urinary tract and near the tip and burns terribly with urination.  The pain causes the urine flow to stop.  It came on several days after anal sex with my wife.  My doctor has ruled out any STD with both swab and blood work. My urine is "unremarkable". My prostate is "boggy". I have been on Doxycycline and Cipro for the past 10 days.  Since I had no improvement with these drugs, the doctor put me on amoxicillian five days ago.  I still have incredible pain with urination.  The opening to my penis is swolen and red and hurts to rub on my underwear. I have low back pain and a small amount of discomfort in the pubic area. I have had this condition in the past.  I was scoped four months ago and no polyps or scarring was found. Doxycycline cleared up the problem then. The doctor is puzzled now.

There has already been a pretty comprehensive evaluation - including being scoped four months ago without evidence of polyps or scarring.  

The only other suggestion I would make would be to ensure the prostate is not infected.  This would include a prostatic massage and culturing off any fluid that is expressed.  This can evaluate whether there is any bacteria that is resistant to the antibiotics you are taking.  

If negative, I would suggest a transrectal ultrasound.  This can image the prostate and evaluate for any abscesses - which would take more than antibiotics to treat.
