Thanks for writing in. It is difficult to confirm, but permanent damage due to compression of nerves can lead to pain in the pelvic region.
I would suggest that you visit a neurologist and urologist for confirmation of cause.

How are you? Pain in the testicle along with pain in the perineum could be due to injury, infection, rarely prostatitis. Do you have any other symptoms like discharge, burning urination, discomfort while ejaculation etc?
I would suggest that you get a urine and semen analysis with culture and prostrate examination done to confirm the cause for these symptoms.
