I would be tested for STDs such as gonorrhea and chlamydia to ensure this isn't the case.
The urinalysis and culture may still be negative even if infection is present.
If a condom was used, the chances of transmitting an STD would be less likely, even during removal/placement of a condom.
If negative, I would consider an scrotal ultrasound to evaluate for epididymitis or a transrectal ultrasound to image the prostate for an abscess.
