There would be several possibilities. The

First-testosterone mc would be any type of genitourinary infection. I would suggest a urinalysis and culture as a first step. Tests for STDs such as gonorrhea and chlamydia should also be done.
If negative, then tests for the prostate should be considered. A digital rectal exam to examine for possible infection can be considered. A scrotal ultrasound to evaluate for epididymitis can also be done.
If infection is not revealed, then anatomical abnormalities can cause pain. A cystoscopy to evaluate any urethral strictures or polyps can be considered.
In any case, I would suggest a urology referral for further evaluation.
