Painful Ejaculation, gel-like beads in fluid

I am 35, healthy, no STD's.  For the past year of so, I have had a weak urine flow, and feelings of never finishing.  Every few days, while urinating, all of a sudden it would feel like too much urine was trying to come out at once, and would cause a pain in my penis that would cause me to stop mid stream.  I could then start again with no pain.  Occasionally, I would get an extreme pain in the tip of my penis, once or twice a year.
Then, in mid July 2007, I was on a roller coaster, about to go down the first huge drop.  My perineum completely spasmed, tightened up into a painful knot, and it killed.  The pain went away within 5 minutes. The next four times I masturbated, it hurt like hell - it felt like I couldn't ejaculate the fluid out and it hurt in the penis.  I had blood work (PSA negative), urine(normal), and testosterone(average).  I was put on Flomax.  

Urine problems better now, but ejaculations continue to hurt.  The longer I go between them, the more they hurt.  Because of the Flomax, my semen quantity was at first reduced, but the last 4 or 5 ejaculates have contained more and more gel-like beads, that resemble tapioca or slightly rounded pieces of rice.  

I went to doctor today, he checked my belly, and did 2-finger sweep, and suggested a referral for a scope. Nothing out of ordinary.
Other symptoms include a spasm in my anus, a pressure like feel to the anus when I'm showering down there that sends an uncomfortable feeling to my penis, occasional ache in the penis, and occasionally a sharp pain in the anus that starts out slow, then gets worse and then goes away.  Once every 1.5 months a few 'stabbing like' pains in my lower right area of my back where I think my kidneys ought to be.  

The doctor requested me to get blood, urine, and a semen sample submitted, which I have done today.  I have to go back to my regular doctor in 9 days.
What is WRONG with me???  Please help.

got a pelvic ultrasound done. results are:

my prostrates normal size, i **** out the right amount of ****, but...

my "right ureter may be dilated - possible dilatation of right distal ureter" leading to ''right hydronephrosis"....

have to go for another ultrasound of my kidneys, and 30 more days of antibiotics.

no idea whats going on...any thoughts?

Your advice is sound and is mostly what I have discovered, plenty of water, no spicy foods and keep it cleaned out but do not over do it either lol!

Base on the symtoms that you two describe is what they call an UTI ( Urinary track Infection)  This is serious but it's also not. It serious because if you don't treated right away it will later damage your kidney. It is not serious because it is easy to treat usually you just take some antibiotic for a few days and leave it alone for a couple of week for it to heal. but in your case an injection might needed since it's been going on for a while. if you need more help email me at my hotmail account and my email is "alreadyfive"

hi.  i am on day 8 or 9 of levaquin, and i have now masturbated 4 times with no pain - back to normal, except for the quantity level (which is because of the flomax).  the gel beads are a lot smaller, and less of them.

peeing is usually (95%) of the time not a problem anymore, but i still KNOW the time its gonna hurt a bit (so is it psychological, i dont know)...

the smell of chlorine is normal.  yellow tinge im sure ull read up on i dont think is a big deal, but i may be wrong....just read up on it

i can honestly say the levaquin is making a difference for sure, and im on it for a total of 30 days, so hopefully, i pray to god, this is the answer...

go for the levaquin, im on one a day pill form 500mg, just for reference.

Hi, I was so happy to find this site today so I could share this info with DH...yes I am a woman but a concerned one at that. My hubby has the same problem but possibly worse? We have been married 17 yrs he has been with no one else but me...been tested no STD's or anything, he has had his prostate checked several times as well as a cat scan and some test where they went inside of his penis as well...Pats has had this pain as long as he can remember its hurts to ejaculate he feels like it wont come our or that its tearing and explains it like razor baldes inside..he also feels like he need to pee more or it wont come out all the way...I feel really bad for him cuz no one can figure it out..and I know it sucks for him to have sex and it doesnt exactly make me feel good to know he is in pain at the end and even kinda makes me feel guilty. I was feeling pretty defensice for yah when that one guy wrote back telling you to chill out etc..apparently he has no idea the pain you really are going through or he would know how serious it is and you CANT just chill out...oh also I have noticed hubbys semen is a little thick or a little gel loke as you are saiying and I have thought that to be od and part of the problem..also is has a slight yellow tinge to it at times..and even an odd smell to me it almost smells like chlorine...do you know what I am talking about? I'm gonna have him read this and maybe he will cut down on some of the spicy things and pops too as the one guy is saying...now that we have heard to do from the Drs before but I know its hard..he use to drink beer like crazy but has quite a yr now in Nov..he only drinks the non alchohol stuff now!! He see's his Dr for another pain in his stomach in another week so maybe we can try the levaquin as well?? Oh and also my DH has pains in his lower back kidney area too..WIERD stuff...Let us know how you are doing please~Chantay

day 6 of levaquin, last 2 ejaculations no pain at all.  holy shxt, thank god....

however, i still have once a day a pain or a spasm, in mid stream while im urinating.  i can sense it somehow that its going to happen...i have to stop urinating, then the spasm stops, and then i can continue....im not sure if its just me being tense and anticipating pain while pissing, or if im expelling shxt in my urine from the antibiotics, or what....

thanks for your comments, i appreciate them alot.  as for diet etc, i do drink alot of water, hardly ever drink, run and lift weights regularly, etc.  i DO put hot pepper flakes on pasta, eat hot sauce though...never given it much thought, and had heard that hot stuff was good for you somehow...i've never had a problem with antibiotics and peeing, so hopefully this levaquin will do it for me....2 scopes?  wow...i can't even think about it

went today to doctors, all urine/blood normal.  semen output was small (i knew that would be because of flomax) and it had low sperm count (which is odd cuz i was tested 8 years ago due to fertility issues with wife, and i was high), so he thinks there may be some infection not showing up on tests, and put me on Levaquin for 30 days.  God I hope thats it.

Friend, I can see that you're worried about whats going on down there and probably spend every minute of your day trying to reassure yourself that everythings ok. You're also a bit like me in that once something goes a bit wrong, you get neurotic about it and it takes over your life.

Lets get this into context, basically your crotch played up on a rollercoaster. Now, I don't know about you but i've never heard of any serious disease listing the symptoms "cramp in crotch when you experience a falling motion".

It sounds to me like a muscle spasm which the rollercoaster drop triggered off. Maybe the prostate was involved, but again it would prove a sensitivity and nothing more.

I'm not just saying it, I honestly believe you'll be fine. As far as ejaculate goes, it can often come out in all sorts of textures but again, beady semen is hardly a symptom of severe disease.

Saying all of this, I also know that no matter how much people tell you everything will be ok, you'll still never really give yourself peace of mind until you know the score! It's your call, see the meds or don't. Maybe see if the symptoms alleviate? Whatever you choose, try to chill.

I've also got weird symptoms going on and am equally inquisitve as to what is going o, so I know what you're going through!

and thank you too for your response, I just didnt know if I'd run out of response chances earlier!  I agree with you...I hope my family doctor gets my results, sees an infection, and I get some antibiotics....I am praying.

appreciate your response though...I just can't seem to find anything on my stupid situation (rollercoaster) and any correlation or anyTHING about my gel like beads in the semen (and what I think is causing the pain in ejaculating it out)...

I assume the doctor I'd go to would be the same one my brother had to go to, and he had it done in the chair, so I assume its the flexible one.  My brother had it done, only because he was urinating a lot (even though he drank alot) and had some discolouration on the tip of his penis....the scope showed nothing wrong with him of course, and I can't believe he went through that for that...I got an STD check 15 years ago (when circumstances didn't warrant it, but the doctor was a new one and eager to I'm sure try it for the first time) and she psychologically scarred me for life doing that test...I'm SO petrified of even contemplating getting that done.

what type of scope does your doctor use? if its one of them old hard one, RUN! they hurt like hell and even require you to be knocked out. A modern flexible fiber scope is really thin and with the anesthetic gell, doesnt hurt at all, i have had to have it done several times and no post scope pain. Never had the hard scope, i read too many warnings online, besides, they are only used by ancient old farts who wont modernize.

Does the rollercoaster spasm incident, and the subsequent painful ejaculations have anything to do with each other - have you ever heard of this before??  From my symptoms, what do you THINK is the problem?  I am deathly afraid and nauseated just from the thought of a scope - I don't think I could even do it, not to mention the pain in urinating after it.  Couldn't the next step be just to put me on antibiotics and see if that resolves it?  If so, what is the best antibiotic?  I have never had a problem with ejaculating before...then the rollercoaster drop spasms my perineum out of control, and the next thing you know it hurts when I ejaculate...PLEASE answer those questions for me, as I don't think I get to post anything else.  Thank you for your time.

I agree with the suggested workup.

Further evaluation of the prostate can be done with a transrectal ultrasound.  I agree with the cystoscopy to evaluate for any strictures or trauma to the lower GU tract.

If kidney damage or disease is suspected, imaging the kidneys can be done with an ultrasound or CT scan.

These options can be discussed with your personal urologist.  Another opinion can be obtained, preferably at a major medical center.

Followup with your personal physician is essential.

obviously your doctor doesn't know for sure so anything anybody else says is pure speculation, but if I were to make a comment from your symptoms, i'd be looking at the possibility of an inflamed prostate.
