Infection should be ruled out. This would include urine tests for a UTI as well as STDs like gonorrhea and chlamydia. The prostate should also be looked at - with a digital rectal exam and prostatic massage. An ultrasound of the prostate can be considered if this is clinically suspected.
If infection is not the cause, you can consider a cystoscopy to look for anatomical abnormalities in the lower GU tract.
