Penis hole smaller or Penis hole skin growing?

I am 13 so this might be puberty but..
last Thursday i went to the restroom when i was going to pee i looked at my penis now it looked fine but when i peed i noticed that my penis hole was small and also it looks like a lip on the right side of my penis hole when i pee..?
is this puberty? is it an infection? i haven't had any sex what so ever, did i masturbate too Much/Hard?
Please help thanks!
