Penis tingle

I am age 50 male, good health but slightly overweight, reasonable stress, active sexually solely with my spouse of 30+ years.  No history of prostate problems. (but Father has prostate cancer). Vasectomy 20 years ago.  Pretty durable high capacity bladder.  No incontinence unless I wait way too long to go.

Recently, I have had an intermittent "tingling" at the tip of my penis which feels like I am "dribbling" a small amount of urine.  The sensation last about 3 seconds then stops.  It repeats again about 5 seconds later.  Sometimes this will continue for an hour or more-- 3 seconds tingling, stop for 5 seconds, tingle again, stop, tingle, stop, etc.  Other times this occurs for only a few minutes then stops.

No fluid leakage, no pain, no discoloration, no unusual smell, no itching, no incontinence, etc. No other symptoms at all, just this annoying intermittent vibration right on the tip of my penis.

Any idea what may be causing this?

If I need a consultation, should I start with my family doctor or go straight to a urologist?

Infection would be something to first rule out.  This can be done via urine tests and culture for STDs and urethritis.  A prostate exam should be done and any prostatic fluid sent for culture.  These tests can be done by your family physician.

If the symptoms continue and the tests not revealing, then a urology referral can be obtained to look further for structural abnormalities - such as urethral polyps or a urethral stricture.  This can be done via a cystoscopy.
