Please Help.

Back in October I had some low back pain.  I went into urgentcare and told them about the low back pain and the frequency of urination during the night.  They ordered a urinalysis and it came back negative.  The doctor performed a DRE and said that due to my reaction that I had prostatitis and gave me Cipro 500mg twice a day for 14 days.  I finished that medication and it seemed to help.
About two weeks after that I had some perinium pain and still felt that I had not gotten over it.  Again a urinalysis was completed and it returned negative.  The doctor then ordered a CT Scan.  Everything came back normal except a slightly enlarged spleen.  He gave me Levaquin 500mg once a day for 30 days and Flomax .4mg once a day.  That really did not do a lot.
I went to the Urologist on December 17, 2008 and he did a urinalysis that also came back negative and had did an exam and found a cyst on my epiditymis and confirmed through an ultrasound that it was in fact on the epiditymis.  He continued with a a DRE and said that it was that the prostate was not swollen or enlarged, but I seemed sensitive to it and that he thought I still had prostititis and gave me Cipro 500mg twice a day again and ordered it for 6 weeks.  Well after two complete days on it, I got a rash on my hand that formed a blister.  He informed me to stop taking the Cipro and the rash started getting better and the blister popped.  Now I'm scheduled to go back in on Monday January 5, 2009 to see the Urologist.  I still have a dull feeling of testicular pain that is just so annoying.
I have no pain during urination.
I have no pain during ejaculation.
I have no problem with my stream.

Please give your input on what you think could be going on.  Thanks in advance.

Went back to the Urologist today and was given Bactrim DS Generic 800/160 for a total of 6 more weeks.  Anyone using Bactrim want to comment on how it's working for you?

There were no white blood cells or blood in the urinalysis at all.  They did not do a culture and I did have a CT Scan done that only showed that I had a slightly enlarged spleen.  I have another appointment on Monday and will ask them to do a culture on my urine.  They said they did not do a culture because there have been no signs of infection in any of the four of my urinalysis tests.

Your symptoms could be due to UTI, or urinary tract infection. The infection could be at any level from bladder to ureter to kidney. It could also be due to a stone in the urinary tract. You must get an ultrasound or CT scan of lower abdomen to rule out stones. Pain in testicles could be due to referred pain of kidney stones. What you need is a urine culture and sensitivity test done. Next time you have this problem, please go to a laboratory and give your urine for culture and sensitivity in a sterile bottle. This will tell you what organism is causing the infection and what antibiotic will kill it permanently. Please do not take any antibiotic without this test because if the organism is not sensitive to the antibiotic, it will only sit quietly for some time and then flare up again.

It is difficult to comment beyond this without examining. Please discuss this with your doctor and get yourself investigated accordingly.  Do let me know if there is any thing else you want to know. Take care!

Also on the second visit to Urgentcare they did a CBC and a PSA test.  The CBC came back completely normal in all fields and the PSA came back 0.4 which is way below the screen for Prostate Cancer.  
I did go to my regular physician about a week ago and he did a prostate massage and then did the urinalysis and leucocytes were perfect and there was no infection, blood, or anything suspicious at all in the urinalysis.  He also checked for diabeties and it was perfect.
