Question # 1 a yeast infection usually infects the skin of the penis. He would have skin lesions if this were the problem. Involvement of the prostate or urethra is usually not seen in non-diabetic men.
# 2/#3  He could have a rare cyst in the prostate causing pain and discharge or an infection with a bacteria which is not sensitive to doxycycline. Trichomonas is also an uncommonly occurring cause of arthritis/prostatitis.  The urologist may need to massage his prostate to obtain fluid to culture and examine microscopically. If this test is  not conclusive, the urologist may request an ultrasound of the prostate to be performed.  The probe is placed in the rectum.  This is very definitive for cyst.  If a cyst is found drainage or removal is usually accomplished with surgery.  The urologist may also recommend a cystoscopy (looking into the bladder with a lighted telescope)  This can help determine if there is any inflammation in the urethra associated with the discharge.
