Prostatitis

by Nateisenough, Jan 20, 2006 12:00AM
I am thirty years old.  I have been having issues with frequent urination for a few years now.  This problem then turned into a number of symptoms in the pelvic region;
-Frequent urination.  Many times it becomes a sudden necessary urge not long after the initial feeling of having to go.
-Waking up several times at night to urinate
-pain and discomfort in the groin, testicles, bladder.  Sometimes it feels like a mild cramp in my bladder and rectum.
-Premature ejaculation.  This has gotten worse recently.
-irritation of the urethra( mostly after ejaculation)
-irritation and sometimes a bit of redness and swelling of the exterior manifestation of the urethra (edema?) The area that runs from the opening of the penis down the split of the head. This symptom concerns me because it seems far removed from the prostate.
-dribbling and a feeling of not having drained my bladder.
- a burning sensation in the urethra
-A change in color of my semen.  this varies from a normal clear color to a greenish-yellow color.  There is no odor associated with the color change.
- However, there is a strong odor to my urine.  It smell almost like coffee sometimes, although I only drink decaff now.
-I had a nocturnal emission recently after I had intercourse the same night.  I haven't had a wet dream for years

I have no discharge, no fever or any other signs of infection.

My doctor said he thinks it is prostatitis.  A prostate exam showed the prostate to be boggy and I expressed a small amount of fluid when he pressed on the prostate.
I was treated for Gonorrhea and Chlamydia as a precaution.
urinalysis came up negative for UTI or bladder infection.
I was treated with a couple of cycles of Cipro.  The symptoms come and go.  Just when I am feeling better things degenerate again.
I don't have insurance yet, and I am afraid to go to the urologist without coverage, but this is really degrading my quality of life.  It is a constant strain on me emotionally and physically.  I have a healthy sex drive, but the pain after ejaculation and the embarrassment of premature ejaculation is altering my libido.
Any insight and advice that you could provide would be greatly appreciated.
Sincerely,
Nathan Hentges

The prostate certainly would be the most reasonable explanation.  Infection should be tested for and treated.  Some cases of prostatitis requires an extended course of antibiotics (sometimes 4-12 weeks).

A prostatic ultrasound can image the anatomy of the prostate, as well as look for any signs of abscess.

You can consider blood tests looking for metabolic diseases that can lead to your symptoms.  This can include blood tests looking for diabetes or thyroid disease.

If the diagnosis remains non-revealing, you can consider a cystoscopy to evaluate for any anatomical abnormality in the lower GU tract.

These options can be discussed with your personal physician.

Followup with your personal physician is essential.
