In 1987, around Feruary, when I lived in England, I had a severe case of bleeding after a foreplay that did not lead to sex.I contacted a urologist who did a cystoscopy on me and found nothing wrong. I was, however, put on antibiotics and the symptom cleared.
In 1999, every year since then, around February and March, I get this sudden bleeding after sex.The bleedings are severe including, clots and blockages from it. For the past three years when this has happened, I have gone to see the doctor who has done the following:
urinalysis
ultrsound of the kidney and bladder
examination of the prostrate through the rectum
PSA test
All the above have always come out negative.
Right now, as I write to you, I have severe blled in my urine sporadically. The first time it happened this time around was after I had sex with my wife. I noticed the bleeding immediately after and when I attempted to urinate Ihad to force it to get the clots out. I have again gone to see my doctor and he ordered a urinalysis that did not show an infection. However, "few" bacteria was seen - I'm not sure if it was in my urine or blood. The doctor has done another urinalysis and a culture of the bacteria that was found.
I have to tell you that with previous cases, the prescription of Cipro has helped to clear the symptom. But this time when I noticed the symptom of extreme tiredness, frequent urination, I asked my doctor for a prescription of Cipro, even though she said I had no infection. I took that for a week and it has not helped.Right now I can't afford to have sex or I will be bleeding profusely. Now, even without sex, the bleeding happens sporadically. What should I do?

Hello - thanks for asking your question.
You note blood in the urine (known as hematuria). PSA, urinalysis, ultrasound, prostate exam all negative. Cystoscopy in 1987 was negative.
The causes vary with age with the most common being inflammation or infection of the prostate or bladder, stones, and, in older patients, malignancy or benign prostatic hyperplasia.
However, there are many cases where the cause of hematuria cannot be found. The combination of negative radiologic examination(s) (which may include IVP, ultrasonography, and/or helical CT scan alone or in combination), negative urine cytology, and negative cystoscopy is usually sufficient to exclude malignancy in the urinary tract.
Initial and then periodic urine cytology and urinalyses should be performed (i.e. at 6, 12, 24 and 36 months). Some physicians also recommend repeat ultrasonography and cystoscopy at one year in high-risk patients with persistent hematuria.
In your case, I would repeat the cystoscopy and perform a urine cytology. If every test remains negative, then periodic urine cytology, urinalyses and/or cystoscopy should be discussed wtih your physician.
Followup with your personal physician is essential.
