Doctor, I get a dull, aching pain in my left testicle after
longterm sexual arousal without ejaculation.  I have been
previously dx as having a spermatocele in the same testicle
about ten years ago that required no intervention.  
The spermatocele has been stable in size over the past ten years.
Should I be concerned about the pain I experience?

No Clyde this ache is very normal and very common experience.  I would continue to perform self testicular exam once a month and alert your physician ASAP if you note any changes.
