: I'm a 39 yr old male, married with two children.  A non-smoker and until recently, a pretty active person.
I have been diagnosed with Ch Prost since mid October.  The Uro found a trace of blood in my urine, but no signs of infection in the fluid sample.
Subsequent visits showed no blood or infection of any kind but my Uro prescribed Septra 500 mg twice a day (allergic to Cipro) for 4 weeks now and have seen little improvement.
Initially the symtoms were a high degree of pain in the perianal area, making it very difficult to sit for any length of time.  Eventually the symptoms had decreased to a burning sensation until recently, when I started feeling pressure and pain in the perianal region again last week.  Ocassionally, I have also experienced pain on the tip of the penis, sometimes its very sensitive.  Recently. while sitting at work for long periods, when I shift on my seat, I  noticed a bit of incontinence, may a few drops worth, when the perianal pain returned.
Are these symtomatic of prostatitis and is it normal for the symptoms to fluctulate like this?
Would mountain biking play a part in this? I ride mostly on weekends from spring to fall.
Are these symptoms related to cancer in any way?
I had a previous bout 6 yrs ago and it lasted over six months before I felt better.
I'm pretty nervous about this and causing quite a bit of anxiety.  Its been very difficult to work. (programmer)
My uro mentioned a Cystoscopy, but I'm very nervous about getting one done, since I had one a few years ago, it was a very unpleasant experience (rigid scope).
Any replies would be greatly appreciated,
PSL

Dear Sal; Thanks for your  questions.
Your symptoms are very typical of nonbacterial prostatitis or prostadynia.  These two diagnosis are made by exclusion of urinary tract infection, bladder or urethral cancer, bladder neck abnormalities, bladder stones, passing kidney stone, interstitial cystitis and urethritis to name a few.  A thorough urological work-up is needed to rule out these other causes.   We have had multiple questions on this forum about prostatitis and it work-up  in the recent past. Please see the answer and questions Bo posted on 2/4/98. Also you may try the Medhelp Site search function and type in prostatitis.  Not a lot is known about the causes of prostadynia  so biking may help, hurt or neither. I would imagine if just seating makes you unconformatable, this would probably aggravate your symptoms.  The only way to know is to try it. I would try it first in moderation.
Prostadynia or non bacterial prostatitis is not associated with prostate or bladder cancer.
Flexible cystoscopy is the state of the art at present and is significantly less uncomfortable.
You are welcome to visit Henry Ford for a urological consultation if travel can be arranged.  If interested, please call 1800-653-6568 for an appointment.
