The causes for testicular pain include:
1)Infection (epididymitis);Inflammation
2)Spermatocele
3)varicocele
4)hydrocele
5)tesicular cancer
6)Torsion
7)Inguinal hernia
In your case, it appears that you may be suffering from chronic epididymitis (infection of part of the testicle). The fact that you have had a good response to antibiotics is encouraging. However, an ultrasound (which you say is scheduled) is a crucial test to help determine if a malignant process is present. In addition, you would require some blood tests to further clarify this problem. These blood tests would include AFP and betaHCG. If your urologist's suspicion is a malignancy of the testicle, you need to follow up without delay. All studies should be performed as soon as possible.
The problem(s) which you are describing need to be evaluated by a urologist. If and when testicular cancer is ruled out as the cause of your problem, your urologist will want to address the other potential reasons (listed above) for your testicular pain.
