Hello,
From the symptoms it looks like it can be a trapped penis or buried penis. Due to obesity fat pad at the base of the penis conceals the penis and penis is hidden or buried within the suprapubic fat pad. This is common in children. For this losing weight or reducing the suprapubic fat by getting a tummy tuck operation during adulthood can help. The other option is surgical release of penis in adulthood.

Also for phimosis, application of a steroid cream to the foreskin up to three times a day for about a month to loosen the adhesive ring. If ballooning during micturition is present, then surgical removal of all or part of the foreskin(circumcision) may be recommended.

It is very difficult to precisely confirm a diagnosis without examination and investigations and the answer is based on the medical information provided. For exact diagnosis, you are requested to consult your doctor. I sincerely hope that helps. Take care and please do keep me posted on how you are doing.

Hi,

At such a young age circumcision is not required. The foreskin can seperate from the glans at a later age. In some males this does happen in their teens.

As for the other issue I don't have an answer for you. You want to consider that with the abnormality, it is possible that the foreskin might be needed for reconstructive surgery.
Circumcision can also make the retracted penis even worse.

All the best,

Ron
