Right Testicle:Lower Inside End:Lump § Dull Pain - My Dr's Opinion

I am 40yr old white male. I woke up 4 days ago with pain in my testicle.  That night I felt around and to my shock noticed a lump (feels like a smooth raised cyst about size of a pea).  It's located at base of my right testicle around the front/center..almost about at the base.   I didn't mess around, saw doctor next day.   He examined me by hand and then I noticed he put light through the testicles with his light instrament.  That's all he did. He told me that "at the bottom of your testicles are vein clumps and he thinks i've got some type of an infection.  He does not think it's cancer tumor.  Said he wants us to watch it and if necessary he'll give me antibiodics to take.  I'm no expert but when i looked on Internet, the area were testest join tubing is on top, not bottom so are there really vein clumps on bottom of testes and could this assessment be factual ? It's now 5th day, pain has not gone away.  I have a 'dull pain in my groin area and i'm very aware of when i walk or touch that area to be gentle as it's pretty sensitive.  The bump is still down there.   Question --- DOES THIS SOUND LIKE A SOUND EXAM APPROACH?  SHOULD I TRUST HIM OR DOES THIS SOUND LIKE I SHOULD BE PUSHING TO SEE A SPECIALIST ASAP? I don't play with myself but know myself enough to know this 'bump' was not there in past.  FYI.. Father had prostate cancer when he was around 56yrs of age.  Grandfather had heart cancer (smoker) at 62.  If that's important for u to know.  Can you please help by providing your medical opinion on what he said ?

With the light test, the doctor is looking for varicoceles (i.e. the vein clumps).  However, with any scrotal mass, I would consider a scrotal ultrasound which can more thoroughly evaluate for any masses, include cysts, varicoceles, or cancer.  If you still have the symptoms, this approach should be considered.

You can also consider a urology referral, although any primary care physician can order the ultrasound.

Followup with your personal physician is essential.
