STD to Urology

Hi,

Initialy i have a STD worries due to protected oral sex about 46days ago.

Now it looks like im having epididytimis, i am having pain on my left testis when i was applying steroid cream on my scrotum due to fungal infection. This was sudden and pain for several hours and the next day, the pain gone.
I went to see doc and he said STD, Doc prescribe doxycline.

Urine test for gonohorrea and clymdia were negative. Blood test for hiv, hep b,c, syphilis were negative.

Radiology detect mild increase in vascularity left testis and epididymis consistent with very mild orchitis. The doc said bacterial infection, not sure what is the cause. Now i am on medication and doing another different urine test.

Do anyone come across such similar condition, do anyone know what problem im having??

Hi Dr J Singh,

I have never been having any kind of this epididymitis orchitis problem before in my entire life, i am not even sure is it a co-incidence right after my encounter.

I only have this one encounter with an escort in my entire life and i am here now with this illness. Test does not seem like STD but is there anything left out, how about NGU? The most bothersome part is not knowing what was the cause? What other factors that can cause this?

I hope that you can help me Dr, thank you.

Hello Dr,

Thank you very much for your insight. Seen my doc today, told him still feeling mild sensation and completed my keflex (cephaxelin) antibiotic.

He said i should try a different antibiotic and it should be okay since is mild sensation. He prescribe augmentin duo forte (amoxcilin and clavulanate) for another 10days.

Hope it helps this time.

Hello,
Orangish red urine can be due to blood in urine which is also called hematuria. Most often it is related to urinary tract problems like kidney stones, inflammation of urinary tract, infections of the urinary tract, prostate enlargement, renal vein thrombosis, certain kidney tumors and polycystic kidney disease, blood coagulation disorders and anticoagulants.

It does not seem to be related to testicular infections causing orchitis.

I hope it helps. Take care and please do keep us posted on how you are doing. Kind regards.

Thank you dr for your comment, i will make an appontment with my doc tmr.

I got another question that i need your expert opinion. Few weeks back i had a wet dream so i quickly ran to the toilet and let my sperm drip off and after a while i feel like peeing so i pee and i saw my urine which is orangey redish that looks like contain blood and feeling really painful while i pee. Does it cause the source of orchitis?

I could not remember the wet dream was before or after my first episode epididymitis. I wish i could remember so it might help in your investigation.

Hello,
What you are having is chronic orchitis and is characterised by inflammation even when there is no infection present. We have to use antibiotics for a longer period of time and other treatment methods include the use of anti-inflammatory medication, such as ibuprofen, painkillers, such as acetaminophen, scrotal support, the application of heat, or nerve blocks.
Rarely, surgery, including removal of a testicle, may be appropriate as a last resort.
I sincerely hope that helps. Take care.

Thanks Dr.

I am still under antibiotics, maybe thats why symptom still persists.

The fungal had stop at one point when i'm not taking any meds, it cameback when i started taking antibiotics. I just started taking Keflex or cephaxelin for my orchitis.

Hello,
Various reasons for recurrent or persistent fungal infection are incomplete treatment, treatment resistance, co-existing dermatitis, diabetes and low immunity. Treatment resistance can be overcome by applying a cream with a combination of antifungals (Clotrimazole 1%, Miconazole nitrate 2%, Tolnaftate 1% and Butenafine hydrochloride 1%). Get her blood sugar tested, fasting, 2 hours after food and if these are high then glycosylated hemoglobin to see overall sugar status in last 3 months.

I sincerely hope that helps. Take care.

I got a question: How long does it takes for fungal infection to heal? I am still getting irritation on my skin where it was infected. I stop using the cream when i feel no irritation , after a while it comes back.

I need some guidance here.

Thank you for your post, it is very helpful. I was trying so hard to figure out what happen. I was thinking of STD but no proof from test and was really scare not knowing the cause, if it was fungal infection then I will be at ease, I told the doc about my fungal infection but he do not think so, he think might be a bug. I am still waiting for my urinary results for any bug. I was really thinking how come i end up like this, if wasn't my encounter I wouldn't end up here. There are 2 possibility that I can think of, I might have caught something that's why I am sick since encounter or I am actually caught nothing and end up like this after antibiotics which lead to side effects from one to another. Anyway I have to be strong and continue to recover, I hope it is not serious illness.

Hi,

I donot believe that it is related to STD.

I used to come down with UTI, kidney infections, epididmytis or orchitis every three to four months. In my case the UTI and kidney infections were most likely due to having a neurogenic bladder.

It is possible that an infection can travel up into the urinary tract ending up spreading into the testicles. Each time that I ended up with epididmytis or orchitis no STD's were present.
I also knew that I did not catch it from my wife having an infection of some kind.
Both of us have had problems with yeast infection, especially in my case as I was treated with many antibiotics. The use of antibiotics often cause yeast infection as the antibiotics kill the needed bacteria in digestive tract. In my case being uncircumcised, the yeast would also develop in the penis.

Epididimytis and orchitis can take a while to clear up. I used to work as a heavy duty mechanic and the infections used to get so bad that I needed to be hospitalized for about 4 to 7 days being on antibiotics.

In 1999, I decided to have a vasectomy in the hope, that by severing the vas, I would prevent infections from traveling into the testes. I ended up with complications and ended up loosing my left testicle. Since 2000, I have not had any infection in the remaining right testicle.

In my case I believe that the infection started in the prostate, bladder and from there ended up infecting the testicles.

I hope that you can get this under control soon. I know how painful it can be, sort of like a constant sensation of having been kicked in the testicles.

All the best.

Ron

Hello,
From your symptoms and investigations, most of the STD’s have been ruled out and it looks like epididymitis. Modes of treatment are antibiotics and anti-inflammatory agents; phytotherapy, anxiolytics, narcotic analgesics, acupuncture and injection therapy.

I suggest you to take the antibiotics as prescribed by your urologist.
I hope it helps. Take care and regards.
