Scan Clarity, Lupron, Cryoablation Success?

Age 74...PSA 24 a year ago...biopsy showed cancer...Lupron shots took PSA currently to 0.5...No scans have been done.

Questions:
Which scan picks up smallest tumors in lymph nodes, CT or MRI?
What is current success rate of cryoablation (I hear Crittenton  Hospital, Rochester, MI has done 700- 800 of them)?
How long does Lupron usually keep PSA to safe levels?
Are penal shots for erection effective when on Lupron?

Dear Bill,
The usual imaging study to look at lymph nodes is a CT scan which is the most optimal modality in terms of  sensitivity and specificity,cost, expediancy and availability.

As far as cryoablation, this is a relatively new procedure, the long term results of which are not established yet.  It has its own set of pros and cons, which you must discuss  with your urologist before committing to this treatment. Some of the post procedure problems would include impotence,irritative bladder and urethral syymptoms as well as rectal symptoms, urethral strictures, fistula formation from the bladder or urethra  through the rectal wall. Some of these are somewhat similar to problems following radiation treatment.  It should not be used if the cncer has already spread outside of the prostate gland to bone or lymph nodes.

The effectiveness of Lupron to prevent biochemical recurrence is variable in different patients.  But as a rule the PSA will eventually begin to rise again within several years , as the cancer cells become more resistant to the effects of Lupron.

The use of penile injections for erectile dysfunction while on Lupron would be effective in most patients.
