In response to your answer of the jelly substance in semen.
Could you specify if this is a symptom of any serious disease or STD?

I am very worried, please let me know if I need to be concerned.

It is unlikely but possible.  I believe the only way to tell would be to actually test for the disease.  Typically a jelly-like substance isn't a typical presentation for STD's - pain on urination or a penile discharge is more common.  

STD's like chlamydia and gonorrhea can be tested for by a simple urine test or urethral swab.  As mentioned below, the semen itself can by analyzed to ensure there is no infection or disease present.  

For further evaluation, you may want to consider a urology referral and possible cystoscopy.  

Followup with your personal physician is essential.
