Should I do a biospy

I have an enlarged prostrate and having a PSA test and digital exam every 6 months.  Digital exam shows no
rough spots other than prostrate is very enlarged.

My last PSA level was 4.81 on 1/18/10

Previous readings were as follows
4/30/99    3.34
10/07/08  3.86
4/08/08    3.40
10/09/07  3.87
4/10/07     3.3

Should I do a biospy; and if so, how should it be intrepreted?

Age is an important point to add in any discussion relating to PSA  levels. Given what you relate about the findings on your digital rectal exam, and I assume that you do not have a urine infection,  a decision would be based solely on your PSA. I would be hesitant to make a decision on the basis of a single determination and I would recommend a repeat PSA after not having had a digital rectal exam nor an orgasm for at least 3 days. If the repeat value is greater than 4, I would recommend a biopsy. If less than 4, I think that it should be repeated in 3 months and if still in the normal range, I'd repeat it in a year along with a digital rectal exam.
As to how to interpret the biopsy, I  trust that the pathologist would know how to do that.
