Sore shaft Numb head

Hey, I have had problems with my penis and testicles for a while now. I’m 17 and in shape. 2 years ago I noticed that my erections were not lasting as long after masturbation and most of the time my penis was soft while masturbating. That got better and I dealt with the lack of pleasure. Last year I noticed that the head of my penis was getting numb and less sensitive. The right side is now fully numb unless I scratch it and the left side of the head is getting there. I am now sore in the shaft of my penis after masturbation and sometimes while I have an erection or am ejaculating. If I massage my penis gently after masturbation it will not hurt as bad and sometimes go away. Also the top of my right testicle hurts sometimes as the other one doesn’t, sometimes it is intense other times it is just damn annoying. It is also raised higher than the left one. I’ve been blood tested, visually and physically checked, had a ultrasound, and have been tested for sexually transmitted diseases and infections by swab (and blood).Nothing has happened in my life that I would have mass amounts of stress about. I’m not a rough person when it comes to masturbation or with a girl so I have no reason to be sore or bruised if I am.  As of now the tip of my penis is numb but can still enjoy masturbation or sexual activities, just not as well or as strong as I would like to. My shaft is still sore on the inside and I still hurt sometimes while masturbating or ejaculating. The right testicle hurts and feels like the pain is coming from the top of the testicle. My bladder sometimes is sore or stings when I have to urinate but the stream is strong and normal just the slight sting. I really need help! Much thanks!
