Dear Dr. Dabagh,
In case of poorly differentiated bladder cancer with distant
metastasisOvarian cancer metastasis
Spleen metastasis - ct scan, as is the case of your father, the only treatment option is chemotherapy. This is usually with a combination regimen with
methotrexateMethotrexate
Methotrexate sodium
Methotrexate sodium, preservative free,
vinblastineVinblastine
Vinblastine sulfate,
doxorubicinDoxorubicin
Doxorubicin hydrochloride
Doxorubicin liposomal, and cis-platin, referred to as M-VAC. The selection criteria for a patient undergoing such therapy would include good
renalAcute kidney failure
Addison’s disease
Adrenal gland biopsy
Adrenalectomy
Cancer - renal pelvis or ureter
Catecholamines - blood
Chronic renal failure
Dialysis
End-stage kidney disease
Kidney diet - dialysis patients
Kidney stones function, and acceptable cardiovascular health.
Usually two or three cycles of chemotherapy are given. In some series complete response in up to 25% to 30% of selected patients have been reported. Another option is combination chemotherapy and radiation.
Varying degrees of success have been reported in select group of patients up to 70%. These are small groups of patients, and therefore no definitive and final decision has been made, regarding the actual effectiveness of treatments in metastatic bladder cancer. I wish your father the best.
