Testicle and back pain

I have been experiencing back pain left side for about 2 yrs.I have had several ab scans,blood tests and xrays for stomach issues (basically resolved stomach issues-nexium)also had elevated liver enzymes which were never really resolved (I have had a liver biopsy)but have come down considerably,they were in the 200-330 range!most of these tests were at Mayo clinic,very extensive testing was done there endoscopies,looked at my pancreas,lots of blood work.etc.also had what mayo called slightly enlarged spleen which did not seem to concern mayo?
Backaches have been consistient on left side for the last 2 years,but now I have been experiencing back pain and testicle pain (left one mainly)and even some pain on right side of back (middle back)I went to internal doctor last week for follow up on liver enzymes and I explained my new testicle pain,(he thought epididymitis)he did full exam head to toe,examined my testicles found nothing unusual.did a rectal exam for prostate and said everything checked fine there too,he wanted some blood work and set up app. for november 21st. I do have a little high cholestrol,low HDL the good cholestrol numbers are a little low (35)and my triglycerides slightly elevated and slightly elevated liver enzymes 40's and 50's ast and alt's so he wants to check that stuff out again.
My concern is this testicle pain which seems to be getting worse! early in the day its not so bad but by early evening my back and testicle are flat out sore and achy,and also pain in lower front and down into my left leg and lower perinium it helps to lay down but its really wearing me mentally and physically.
I am 41 yrs.old about 30 lbs. overweight other than the last couple yrs. have been basically pretty healthy,no cancer in family history.
questions are as follow:
Does this sound like testicular cancer?
Should I call back and get scheduled with a urologists before my follow up with G.I doc?
What does this sound like to you?and what are your reccomendations?

To answer your questions:
1) Testicular cancer is possible, but there are a host of other possibilities - including prostatitis, groin strain, hernia or infection. A scrotal ultrasound would be the test to rule out testicular cancer.
2/3) I would suggest a urology referral. Sometimes prostatitis can be difficult to detect. A transrectal ultrasound can be considered to image the prostate and evaluate for any abscesses. Another consideration would be a pelvic CT scan which can evaluate for hernias.
Followup with your personal physician is essential.
