Testicle pain steming from between the legs

  I am a bicycle rider that rides 2 to 4 times a week from 1 to 2.5
  hours.  On a ride I experienced pain that came from my lower back
  down to my left testicle.  It has been 1.5 weeks since this has
  happened.  Initially the testicle lifted and was sore, but has
  since dropped back down and I feel pain at the dorsal end of
  the scrotum.  I was wondering if this damage could have occured
  from my bicycling (I sat on the seat once after the incident (but
  did not ride) and it was sore)?  Secondly, what was damage?  A
  nerve or blood supply?  I went to a doctor and after an
  examination he suspected I just hurt myself bicycling.  He
  ordered an urinalysis and ultrasound of the left testicle.  
  The urinalysis and ultrasound had no significant findings.

Thank you for your letter.  Riding a bicycle for  a long time has been associated with few urological problems. For example an elevated PSA was found in some patients who were ardent bikers. Some experts believe biking could also cause erectile dysfunction by causing decreased blood flow to the penis. You describe a pain that started in your back  and radiated to the testicle. This could also be a neuralgic pain that may have started from the stooped position one assumes while biking. Occasionally a small renal stone passing down the ureter can cause similar pain. It could also simply be a bruising of the area under the scrotum from long hours of biking. Since the scrotal ultrasound and the urinalysis were normal I do not think there is any thing to worry about. Perhaps you need to look at the type of  the bike seat if this becomes a recurrent problem. Thank you.
